Altus Air Force Base

Fire Department NFPA Testing

Statement of Work

1. SCOPE OF WORK: The contractor will provide the 97 CES/CEF all labor, tools, equipment, test equipment, personal protective clothing and equipment, materials, transportation and any other incidentals necessary to travel to Altus AFB, OK to perform the NFPA

Testing outlined below.

a. Perform hose testing IAW NFPA 1962 – Standard on Fire Hose.

b. Unload all hose to be tested.

c. Perform a visual inspection of each hose to identify any physical damage.

d. Perform a hydrostatic pressure test.

e. Perform a hydrostatic pressure test on all nozzles and appliances IAW NFPA 1962 during this time as well.

f. Ensure that all hose which fails any part of the inspection is marked and removed from service.

g. Mark both ends of each section of hose with stencils provided by 97 CES/CEF and black spray paint provided by the contractor.

h. Reload all hose to department specifications with the exception of “hotel packs”.

i. Perform ground ladder testing IAW NFPA 1932—Standard on Ground Ladders.

j. Perform vehicle mounted equipment testing on all pumps and pump systems

IAW NFPA 1911.

k. Provide both digital and hard copy of all test results within 10 days of completion to include: Date of test, service test pressures, hose diameter, length, type, identification number, location (which apparatus the equipment is assigned to) and pass/fail for each piece of hose, nozzle, appliance or ground ladder as required per NFPA 1962 and 1932.

l. Provide both digital and hard copy of all test results within 10 days of completion to include: Date of test and all pertinent information as required by NFPA 1911 for vehicle mounted equipment.

2. Standards and Codes: The contractor shall comply with all Federal, State and local regulations pertaining to the required work including, but not limited to:

a. National Fire Protection Association (NFPA)

b. Occupational Safety and Health Administration (OSHA)

3. Delivery Schedule: Testing to be completed with March‐April 2021.

4. SECURITY REQUIREMENTS: The Contractor shall be responsible to gain access to Altus

Air Force Base through the contract. The Contractor shall coordinate with the CO and

Fire Department to gain access to Altus AFB.

a. While on Altus AFB, the Contractor shall comply with Altus AFB security measures. These regulations are available for review in the contracting office.

Security and base access requirements are contained in clause 5352.242‐9000 “Contractor Access to Air Force Installations.” Contract employees will be required to obtain and display identification badges.

b. Contractor employees shall provide information to assist in the completion of background checks for employees requiring access to Altus AFB. This information shall consist of, but is not limited to: full name (first, middle, last), social security number, date of birth, address, driver’s license number and state of issue. This information will be protected under the procedures listed in the Privacy Act of 1974. A background check must be completed prior to unescorted access while on Altus AFB. If the background check reveals a Contractor employee or prospective employee with an adverse criminal history or as a potential threat to the health, safety, operational mission of the installation, then the Government reserves the right to restrict or deny access to Altus AFB for that individual.

c. Contractor Badges. The Contractor is responsible for tracking and retrieving badges from employees upon their separation, termination and/or completion of business on Altus AFB, OK. Badges from employees who are no longer employed on the contract must be retrieved and turned in to the COR within 30 days of the individual’s separation or termination. Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access.

Businesses may lose base pass privileges if they fail to control use of the badges or do not return them upon completion of use. Failure to control or turn in badges may result in contract termination or a decision not to exercise the option periods of this contract.

5. ANTITERRORISM. Altus Air Force Base has an Antiterrorism program that is responsible for securing and protecting the base and its personnel from terrorist attacks. All contractor personnel and sub‐contractors are required to obey all orders from Security Forces and adhere to all security measures implemented during Force Protection Condition (FPCON) changes and Random Antiterrorism Measures (RAM’s). Security Forces members may conduct inspections of your company as well as your sub‐contractors while operating on the installation. Altus Air Force Base is currently in FPCON Bravo, meaning an increased and more predictable threat of terrorist activity exists. All contractor personnel and sub‐contractors are expected to report any suspicious persons, activities, packages, or items you see while conducting work on Altus Air Force Base to base Security Forces. Suspicious activities could include personnel conducting surveillance, unauthorized personnel requesting access, or information probing pertaining to security operations or assets on the installation. In the event of an emergency, there are multiple mass warning notification systems on Altus Air Force

Base (AtHoc, weather radios, telephone notification, and giant voice system). Any contractor personnel or sub‐contractor that obtains a Common Access Card (CAC) MUST complete the Antiterrorism Level I training within 30 days of receiving your CAC (see paragraph 4.8.1.) Altus AFB has an Antiterrorism binder available for review at the Visitor Control Center, building 428. If there are any questions in regards to Antiterrorism, contact 580‐481‐7350.

6. General Provisions:

a. Notification: The contractor shall coordinate all work with the end‐user POC. The contactor shall provide notice of five days prior to work starting to allow end‐ user time to prepare equipment getting tested.

7. Safety: The contractor shall maintain a safe and healthful working condition for all contractor personnel and must ensure conformance with standards developed under United States Public Law 91‐596, OSHA standards, American National Standards Institute and NFPA.

8. Mishap Reporting: Report all mishaps involving any person sustaining injury or illness from contractor operations and all mishaps involving Government property or equipment damaged by the contractor to the end‐user.

9. Environmental: Contractor is required to comply with all environmental standards as required by Altus AFB 97 CES/CEIE. They can be contacted at 580‐481‐6221.
