About this file

This is a Confined Space Entry Safety Checklist document for wastewater treatment facilities operated by the State of New Hampshire, specifically covering operations at the Franklin Compound, Laconia Compound, and associated pump stations and siphon locations throughout the Winnipesaukee River Basin service area. The document establishes safety protocols and identifies potentially hazardous confined spaces requiring permits, checklists, and specialized equipment before entry. The checklist applies to three primary facility locations: the Franklin Compound (including OPS Building & Tunnels, Primary and Secondary Treatment areas, Disinfection Facilities, Solids Handling Areas, and Plant Grounds); the Laconia Compound (including flow channels, pump stations, clarifiers, and contact tanks); and Other Locations (including sewer and drain manholes, catch basins, metering chambers, siphon chambers, and pump stations at Smith Road, Lower Bay Road, and Belmont). All confined space entries must comply with 29 CFR 1910.146(b) and LAB 1402.02 regulations.

Safety requirements vary by location and hazard type, with specific precautions mandated for atmospheres testing positive for low oxygen, toxic gases (particularly hydrogen sulfide), fire and explosion risks, and engulfment hazards. Standard entry requirements include gas meters, safety attendants, harnesses, tripods or davit arms, blowers for ventilation, hard hats, and the two-man rule. Critical restrictions prohibit blower use when Lower Explosive Limit (LEL) readings reach or exceed 100 percent, as introducing air could trigger explosions. Certain tanks (clarifiers, aeration tanks, and contact tanks) require continuous atmospheric monitoring and attendants only during cleaning operations, but become non-confined spaces afterward if standards are met. Digesters entered after cleaning or painting require mechanical ventilation, with subsequent entries requiring continuous gas monitoring and attendants. Specific pump station locations are designated as non-atmospheric hazards under certain conditions, allowing discretionary use of safety equipment based on attendant judgment.

PRECAUTIONS

(1) Clarifiers, contact Tanks and aeration tanks being taken out of service (i.e., being cleaned) they shall be entered only with gas meters and attendants. After cleaning, these tanks do not have the potential to contain a hazardous atmosphere; therefore they are not confined spaces and they may be entered without atmospheric monitoring or attendant as long as 2 workers are present at all times.

Potentially Hazardous Places Including CONFINED SPACES AS PER 29 CFR 1910.146(b) and LAB 1402.02

(5) On Primary Digesters with ramps. When making

Operations inspections or performing minor adjustments requiring less than 15 minutes, no checklist is required. For

Maintenance Inspections or other work expected to last longer than 15 minutes, a checklist is required. Using the ladder on Digesters requires a check list. (Confined Space)

(4) When set up of tripod is not feasible, use of harness and lanyard may be substituted. Whenever lanyard and harness are used, 2 attendants must be present.

(2) Digesters being entered after cleaning or painting have the potential for low oxygen, toxic fumes, etc. and shall be entered only with mechanical ventilation (i.e. blowers).

Subsequent entry to these tanks may be made without mechanical ventilation, but continuous gas monitoring and an attendant are required. Once at the floor of the digester, an entrant may disconnect from the tripod cable.

(3) In addition to locations noted, blowers shall be employed in any location which, upon testing shows low oxygen, or hydrogen sulfide. BLOWERS MUST NEVER BE USED in a situation where LEL is reading at or greater than 100 percent, since adding air to such an atmosphere will increase the potential for explosion.

(5) For the dry sides at Smith Road and Lower Bay Road Pump Stations: These locations have been determined to not pose an atmospheric hazard, thus a confined space entry permit is not required. The use of a davit arm/ harness & tripod may be implemented under the discretion of the attendant working at these locations if the attendant does not feel comfortable entering these locations without additional safety equipment.

(4) When set up of tripod is not feasible, use of harness and lanyard may be substituted with 2 attendants present.

(3) Blowers shall be employed in any location which, upon testing, shows oxygen, or hydrogen sulfide. Blowers shall also be used at Pump Stations

(p.s.) when the installed ventilation system is non-functional. BLOWERS MUST NEVER BE USED in a situation where LEL is reading at or near 100 percent, since adding air to such an atmosphere will increase the potential for explosion. Oxygen deficiency is not a potential hazard provided that ventilation system is kept in operation.

Potentially Hazardous Places Including CONFINED SPACES AS PER 29 CFR 1910.146(b) and LAB 1402.02

OTHER LOCATIONS

(1) Traffic hazards exist primarily for the entry attendant(s), and for vehicles when structures are located on the traveled way.

(2) The portion of the structure not accessible by stairs would be entered only when the wet well has been drained.
