About this file

This is a Request for Proposal (RFP) financial proposal template for a pharmaceutical benefit manager (PBM) contract covering three contract years: CY2027, CY2028, and CY2029. The RFP outlines detailed requirements for administrative fees, clinical services, and pricing guarantees across multiple pharmacy channels including retail 30-day, retail 90-day (if applicable), mail order, and specialty pharmacy services. Bidders must provide comprehensive pricing proposals that include base administration fees, clinical program fees, and itemized service offerings. The proposal requires specification of the proposed broad retail network, exclusive specialty pharmacy, and formulary. Key service areas include account management, customer service, formulary administration, pharmacy network management, utilization management edits, prior authorization processing at multiple review levels, appeals processing, and specialized programs such as fraud waste and abuse interventions and opioid management programs.

The pricing structure requires transparent pass-through pricing for retail networks and acquisition cost plus pricing with underlying discount guarantees for mail order and specialty channels. Bidders must provide minimum rebate guarantees as a percentage of total AWP (Average Wholesale Price) for brand drugs and maximum dispensing fee guarantees per script across all channels and contract years. The proposal mandates 100 percent pass-through of rebates and requires bidders to submit estimated costs based on projected ingredient costs, dispensing fees, administrative fees, member cost sharing, and rebate projections. Bidders must also identify excluded drugs (other than DAW 5 house generics) from pricing guarantees, provide a specialty drug list with pricing details, and propose either per-unit or PMPM (Per Member Per Month) cost guarantees with an annual maximum PMPM cost guarantee. For CY2028 and CY2029, if PMPM guarantees cannot be provided, bidders may propose a guaranteed maximum annual PMPM trend rate instead.

Confirm the methodology that you are using as the basis for your Acquisition Cost Plus model (e.g., % of AWP, % of WAC, % of WAC by GPI-14, etc.):

In this scenario, Bidders should assume an exclusive PBM specialty arrangment for all specialty drugs.

2.2.2 Discount Guarantee

Discount Guarantee

Directions:The State is interested in transparent (pass-through) for retail network and Acquisition Cost Plus pricing with underlying discount guarantees, by channel and drug type

In this scenario, Bidders should assume an exclusive mail order arrangment for non-specialty drugs.

Note:

Retail (non-speciaty) should be quoted on a 1-31 day supply basis Mail Order (non-specialty) should be quoted on a 1-90 day supply basis. Note: the State does not currently allow for 90 day fills at Retail, but would like to understand how it could potentially be cost effective for the State?

Under the State's current plan design, specialty drugs are only available via the PBM's specialty pharmacy only and are limited to a 30 day supply

Confirm the methodology that you are using as the basis for your Mail Order Acquisition Cost Plus model (e.g., % of AWP, % of WAC, % of WAC by GPI-14, etc.):
