Vendors must provide three references for similar services rendered within the last three years, the counselor's resume, a copy of the current Illinois license, a record security process document, and proof of Professional Liability Insurance of $1 million per claim and $3 million aggregate. Subcontracting is not permitted. All quoted prices must remain valid for 30 days from submission, with no price increases accepted; any price decreases will benefit ISAC. Vendors must establish an appointment system for employee scheduling with fulfillment within one week of request and submit bi-weekly status reports to the Director of Human Resources without disclosing employee identities or appointment reasons.
