Attachment 1 – Questionnaire
General
1. Name of Company, SAM.gov Unique Entity Identifier (UEI) number, and Cage Code.
2. Point of contact name, phone number and e-mail address.
3. Size of business according to North American Industry Classification System (NAICS) Code 334220 Radio and Television Broadcasting and Wireless Communications Equipment Manufacturing/ Size Standard 1,250.
Solution
1. What product(s) do you offer that might fill this requirement? (Describe system capabilities and performance, environmental specifications, physical dimensions, and power requirements).
2. Does your product provide Satellite Compass Heading?
3. Is the proposed product a “commercial product” as defined in the Federal Acquisition Regulation (FAR)? (Reference: Part 2 - Definitions of Words and Terms | Acquisition.GOV)
4. Do you offer a commercial product that could be modified to fulfill this need?
5. Is this product available under a current GSA FSS/MAS Contract or do you currently have a contract for this product with any other Federal Agency?
End of Manufacture/End of Support
1. What is the anticipated end of manufacturing and end of support dates for your proposed solution/product?
2. Are you willing to enter into an Indefinite Delivery Indefinite Quantity (IDIQ) Contract with a five (5) year Ordering Period?
