PERFORMANCE WORK STATEMENT (PWS) FOR

CRANES AND HOISTS PREVENTIVE MAINTENANCE AND REPAIR SERVICES AT TRAVIS AIR FORCE BASE, CALIFORNIA

DESCRIPTION OF SERVICE. The Contractor shall perform Repair Services, Annual Load Testing Inspection and Quarterly Inspections for Cranes and Hoists at Travis Air Force Base, California in accordance with all applicable laws, regulations, standards, instructions, and commercial practices described in this Performance Work Statement (PWS) with a focus on safety and customer service. The Contractor shall have and maintain all licenses and certificates required to perform all work requirements in the PWS (par 4.6.1.). The Contractor is encouraged and expected to use innovative approaches to efficiently and effectively accomplish PWS requirements in a timely manner that fosters pride and ownership in the work performed. The Contractor shall, except as specified in the PWS as Government furnished property or services, provide everything needed to perform Cranes and Hoists inspections and repair services at Travis Air Force Base (TAFB) as defined in the PWS. Crane and Hoist Services will be performed throughout Travis AFB, CA. The estimated workload and crane/hoists locations are listed in Appendix A, entitled Workload Estimate and Data. All work performed by the Contractor in the PWS shall conform to the applicable latest editions of the following specifications and standards but not limited to:

SPECIFIC FUNCTIONS.

Maintenance Services. The contractor shall perform three (3) quarterly inspections and one (1) annual load testing inspection of all cranes and hoists to ensure reliable and continuous safe operation of all cranes & hoists. The work shall be in accordance with the PWS and shall be in accordance with the manufacturer’s instructions or standards in the PWS. Some repairs or replacements of parts may require load tests or recertification by the Contractor dependent on the type of repairs or parts replaced. The additional repairs required, testing, and/or recertification will use labor and/or parts line items.

Maintenance Schedules. The Contractor will establish an applicable schedule for all inspections and update it for each option year exercised. The schedule shall be submitted to the Contracting Officer Representative (COR) no later than fifteen (15) business days after contract award. Aircraft hangar availability may affect proposed schedule. The Contractor and the COR will coordinate such occurrences. The approved schedule may not be changed without the written approval of the COR. The Contractor will coordinate with the COR seven (7) business days prior to the inspections to avoid any interference with work within the facilities. Upon completion of repairs, inspections and load tests, the Contractor shall provide an inspection report as stated in para 1.3.1. All weight tests shall be conducted using certified solid weights.

Maintenance- Annual Load Testing and Quarterly Inspections. The Contractor shall perform annual load test inspection and quarterly inspections for the equipment listed in Appendix A. the Government reserves the right to modify the contract by adding or removing cranes and hoists from the equipment list in Appendix A. Changes shall be approved in writing by the Contracting Officer (CO) through the modification process.

Communication. Contractor shall utilize phone, email and postal services in order to be responsive and timely to all requirements of the cranes and hoists PWS.

EQUIPMENT REPAIRS AND SERVICE CALLS.

Repair Calls. The Contractor shall respond to repair service calls on-site for inoperative cranes and hoists, 7 days a week, 24 hours per day, within six (6) hours after notification by the COR. The Contractor shall survey the condition of the cranes and hoists, and provide to the COR a written repair estimate as specified in paragraph 1.3.2. The Contractor shall commence repair work after notification from the COR. Downtime of the cranes and hoists shall be kept to an absolute minimum and the customer shall be informed of findings and estimation of downtime by the COR. The Contractor shall prepare and submit a written repair report as specified in paragraph 1.3.3.

Additional Load Test. Whenever the Contractor responds to emergency service calls and removes, repairs, installs, or replaces a component in the Load Controlling Path (LCP) of any crane and/or hoist, the Contractor shall notify the COR for approval prior to conducting a load test to ensure the unit meets all manufacture’s specifications. Upon written approval from the Contracting Officer or COR, the Contractor will perform a load test and provide new certification documents, recording all details of the load test. The additional cost, if any, for this load test will be included on the repair estimate. This additional load test does not replace the regular annual load test for the equipment and cannot be used to change the date of the next scheduled annual load test for the repaired lift equipment.

Identification Markings. The Contractor shall label, to be visible from ground level, the designated hoist number, the rated capacity, month and year of the inspection on each unit during the annual inspections. Additionally, the Contractor will remove any markings which have been superseded or are erroneous.

Warranty on Service Call Repairs. The Contractor shall warranty all parts, materials and workmanship provided and performed under the PWS for a period recommended by the manufacture and provide the warranty in writing to the COR. Warranty replacement will be done by the Contractor at no charge to the Government. The effective date on all warranties shall begin at the time that the crane, hoist or part is made operational. The Contractor shall ensure the crane or hoist operates as designed and is fully functional after completion of service or repair.

Routine Repairs and Services. The contractor and the COR will coordinate and schedule repairs for routine service calls. The contractor shall respond within 2 hours of notification by the COR and be on-site within 24 hours of notification. Routine Service hours are from 7:00 a.m. to 4:00 p.m., Monday through Friday.

Emergency Repairs and Services. Emergency services are required when item or items of equipment listed in Appendix A require repair outside of Routine Service Hours that were outlined in paragraph 1.2.5. The contractor and the COR will coordinate and schedule repairs for Emergency service calls. The contractor shall respond within one hour after notification by the COR and be on-site within 2 hours of notification. Emergency Service after duty hours are 4:00 P.m. to 7:00 a.m., Monday through Friday, all day Saturdays, Sundays, and federal holidays.

Repair and Services Reports/Receipts. The contractor shall provide a professional company inspection form to the COR with inspection findings, preventative maintenance and servicing of all equipment, parts, material cost and labor services. The service reports shall be provided to the COR within three business days from the date of service. Reports may be hand- carried, emailed as a MS Word, Excel, Adobe (pdf) to the COR.

Reimbursable Parts/Equipment. The contractor shall obtain all parts necessary for the performance of this contract. For the purpose of parts reimbursement, the contractor must have authorization to purchase these parts from the COR and if authorized the contractor shall submit the original receipts and supporting shipping costs. The contractor shall obtain competition to the maximum extent possible for all parts (individual or aggregate).

CONTRACTOR FURNISHED REPORTS. Ensure that all records created as a result of the processes prescribed are maintained in accordance with AFMAN 33-363, management of records, and disposed of IAW the Air Force Records Disposition Schedule (RDS) located at https://www.my.af.mil/afrims/afrims/afrims/rims.cfm. AFI 33-332, Air Force Privacy and Civil Liberties Program and DOD5400.7-R_AFMAN 33-302, Freedom of Information Act Program must also be followed.

Inspection Reports. The Contractor shall prepare and submit a written report within ten

(10) Business days after each of the annual and quarterly inspections are complete. The report shall identify each type of crane and hoist, the location (building number), maintenance work performed, repairs needed, date of inspection, name of the technician(s) who actually performed the work, and the overall condition of the equipment. Whenever a load test is performed, the Contractor shall also provide the load test certification with the inspection report. If the needed repairs annotated in the inspection report require work considered over and above routine maintenance work, the Contractor shall provide a written estimate as stated in paragraph 1.3.2. The Contractor shall provide the repair estimate to the CO and a copy to the COR and a professional opinion when the repair work will be completed. For annual load test failures, the Contractor shall immediately notify the COR that the crane and hoist failed the test and a written repair estimate shall be provided within two (2) business days.

1.3.2 Written Repair Estimate(s). If a crane or hoist cannot be repaired at the time of the emergency site visit due to non-availability of parts or prohibitive costs, the Contractor will notify the COR and CO and provide a cost estimate at that time or by the next business day. The written estimate is submitted to the COR and CO and will be based on returning crane or hoist to a fully functional condition. This written estimate will be provided at no cost to the Government and shall include the estimated cost of parts, labor, man lift, fork lift, hours to perform repairs, the projected equipment downtime, each hoist ID, the location, serial number, make and model and the rated capacity of the hoist, maintenance work to be performed, recommended repairs, date of inspection, and name of inspector. The Contractor shall only make the needed repairs or replacements after written approval from the CO. The Government shall not be responsible for work performed without written approval from CO. For load control path failures, the Contractor shall immediately notify the CO and COR that the crane or hoist failed the test, and a Written repair estimate shall be provided the government within (3) three business days.

1.3.3. Written Repair Report(s). The Contractor shall submit a written repair report to the COR within (5) five business days after repairs are completed. The report shall include the date and time of the service call, the crane ID, make, model, rated capacity of the hoist, locations (building number), overall condition of the hoist prior to repairs, the repairs performed to include replacement parts, labor breakout, actual equipment downtime, name of the technician performing the repairs and the signature of the crane/hoist point of contact (POC) or COR.

SERVICE SUMMARY. The Service Summary (SS) captures the service requirements that can be measured and are PWS requirements. The SS includes performance objectives that describe the desired end result or outcome the Contractor shall achieve. The Performance Thresholds state the minimum acceptable level of the performance in terms of quality, timeliness, and quantity in realistic and achievable standards. These thresholds are critical to mission success. The PWS paragraphs reference the details of the Performance Objective. Any work missed or repair fails will be documented and needs to be reworked by the contract. All related documentation will remain as a permanent record throughout the contract.

2.1. Note: 1) A Performance Assessment Report (PAR) will be used to document poor performance falling below performance thresholds. Note: 2) Unresolved customer complaints or falling below performance threshold levels and repeated unacceptable performance will result in poor performance evaluation(s) and/or cost reduction(s).

HOURS OF OPERATION.

3.1. Hours of Operation. The normal hours of operation are 7:00 A.M. to 4:00 P.M., Monday through Friday. The Contractor is not required to provide repairs and inspection services for any crane and hoists on the ten (10) Federal Holidays or weekends with the exception of an emergency call.

Federal holidays are as follows: New Year’s Day - 1 January Martin Luther King Day - 3rd Monday in January Washington’s Birthday - 3rd Monday in February Memorial Day - last Monday in May Independence Day - 4 July Labor Day - 1st Monday in September Columbus Day - 2nd Monday in October Veteran’s Day - 11 November Thanksgiving Day - 4th Thursday in November Christmas Day - 25 December If a scheduled service falls on an observed Federal Holiday, service will be performed on the next workday, (i.e., if a holiday falls on Monday, service will be provided on Tuesday). Due to mission requirements, work will be scheduled in advance.

GENERAL INFORMATION:

Quality Control Plan. The contractor shall develop and maintain a quality control program to ensure maintenance and repair services are performed in accordance with applicable standards as stated in paragraph 1 of the PWS and the manufacturer’s instructions. The program shall include procedures to identify and prevent defective services. A copy of the plan shall be furnished to the COR and CO 15 business days after contract award.

Quality Assurance. The government will evaluate the contractor’s performance by a COR or government representative who will monitor performance through on-site inspections, receipt of complaints from base personnel and compliance with this PWS. The government reserves the right to inspect each task as completed.

Hours of Operations. Normal duty hours are Monday through Friday, 7:00 A.M. to 4:00 P.M., excluding the 10 federally observed holidays (New Year’s Day, Martin Luther King Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving and Christmas). Federal holidays that occur on Saturday are observed on the Friday before the holiday. Federal holidays that occur on Sunday are observed on the Monday following the holiday. If times outside of regular duty hours are required, the contractor must coordinate with the CO and COR.

Work Approval and Reporting Procedures. The contractor shall provide contact information for the project manager assigned to this contract for routine and emergency services. All work performed under this PWS must first be approved by the COR. The contractor’s official estimate shall be documented sufficiently to identify the system to be worked, estimated repairs, parts, labor, and costs, if any.

Affirmative Procurement. In order to comply with the affirmative procurement requirements of Section 6002 of Resource Conservation and Recovery Act of 1976 and Executive Order 13101, the Government requires the use of the recycled and recovered materials and products identified in the Environmental Protection Agency’s (“EPA’s) Comprehensive Procurement Guidelines. These materials and products must meet the requirements of this PWS, must meet the manufacturer’s specifications, must not delay the progress of the work, and must not be cost prohibitive. EPA guideline items are seen as the minimum that should be considered when evaluating recyclable materials. Other materials and products not listed, but commonly used in industry outside of the government, should also be considered. The Contractor shall provide, to the maximum extent practicable, materials with recovered material content at the levels recommended by the EPA’s CPG website; http://www.epa.gov/cpg/products. EPA designated recycled and recovered materials and products to possibly be considered in the PWS are lubricants (grease, oils, etc.). Cost of parts will also be evaluated through market research.

CONTRACTOR SPECIFIC REQUIREMENTS:

The Contractor shall be required to have a current State of California Contractor License, accredited by Cal-OSHA with a valid CA number, and required to have current Bridge and Gentry Crane certification for each technician who will be working at Travis AFB. Current Certifications will be submitted to the CO and the COR.

ENVIRONMENTAL AND SAFETY CONTROLS:

Environmental Management System (EMS) Policy. TAFB is committed to environmental stewardship while sustaining our military mission. We are dedicated to reducing environmental impacts, sustaining environmental compliance, and increasing the efficient use of resources. In support of our environmental policy, we will: Protect human health and natural resources; Incorporate environmental commitments into applicable policies and programs; Encourage an environmental culture across all levels of management; Enhance relations with the local and regulatory communities; Partner with all stakeholders to promote resource conservation; Achieve continuous improvements in environmental performance.

Upon contract award, the COR will notify the EMS Coordinator to arrange EMS training. The training consists of a five minute Flash video, which will be provided to the Contractor on a CD. All staff who will be working on the base must receive this EMS training. Within 7 days of the start date, the Contractor shall provide the EMS Coordinator with a list of names of all employees who viewed the training as well as the date training was completed. In addition, the Contractor shall ensure that their employees are aware of the roles and responsibilities identified by the environmental management system and how these requirements affect their work performed under this contract.

The Contractor shall perform work under this contract consistent with TAFB EMS and in a manner that conforms to all applicable environmental laws and regulations. The Contractor shall provide information as necessary to demonstrate conformance with these requirements. In the event an environmental nonconformance or noncompliance associated with the contracted services is identified, the contractor shall take corrective action immediately or based on the time schedule established by the EMS Coordinator, 60 CES/CEA, 411 Airman Drive (B570), Travis AFB, CA 94535, phone 707-424-8225 or 707-424-3739. The contractor shall participate in the Base HAZMAT program as required by AFI 32-7086, AMC Supplement 1. Participation shall include:

Designation of a single individual who has experience and knowledge of HAZMAT issues;

Provision of all HAZMAT to be used or stored on base; (N/A)

Limited storage of HAZMAT on base; (N/A)

Coordination of all HAZMAT used or stored on base from 60 CES/CEAN;

Tracking daily usage of all HAZMAT used or stored on base; (N/A)

Implementation of Best Management Practices (Bumps) to prevent spills and other releases to the environmental to meet NFPA, OSHA, and RCRA requirements;

Participation in an accurate exit inventory of materials used on base at the end of the contract. (N/A)

The contractor shall store all HAZMAT in a designated HAZMAT storage area.

The Government will quarterly/ annually, etc. audit the HAZMAT storage area to ensure compliance.

Hazardous Waste Management. The contractor is responsible for the analysis, characterization, proper handling and disposal of hazardous waste generated by their operation. Management shall be in accordance with Title 22 of the California Code of Regulations and the Travis AFB Hazardous Waste Management Plan. The contractor is subject to the Solano County Department of Resource Management, California Department of Toxic Substances Control, Federal EPA and 60 CES/CEAN audits while working on Travis AFB. If hazardous wastes are generated, the contractor shall establish a Satellite Accumulation Point with the approval of 60 CES/CEAN and provide certification of proper training to handle hazardous waste. Hazardous waste shall be placed in secured, properly labeled containers provided by the contractor. The contractor shall anticipate 60 CES/CEAN audits to ensure their storage area is in compliance with requirements.

Under no circumstances will the contractor remove any hazardous waste from Travis Air Force Base without ensuring complete and full compliance with all applicable existing environmental laws and regulations. All shipments must be prepared in accordance with Department of Transportation Regulations (40 CFR 172).

Prior to removing, transporting or disposing of any hazardous waste from the base, the contractor shall forward hazardous waste manifests, land disposal restrictions, waste profiles, laboratory analyses, and Safety Data Sheets to 60 CES/CEAN to obtain a review and/or signature as required. Hazardous waste treatment is not permitted on Travis AFB unless approved and monitored by 60 CES/CEAN.

Notification of Environmental Spills. If the contractor spills or releases any hazardous substance that contacts or has the potential to contact water, soil or any drain (sanitary or storm) the contractor shall call 911 (4-4911 if utilizing telephone on base) then the CO immediately. The contractor shall be liable for reimbursement of containment and environmental clean-up of the spill or release of such substance.

Wastewater. Wastewater is any unwanted water (contaminated storm water, any water not meeting the definition of storm water, non-storm water, ground water, unused potable water, water generated as a result of rinsing or cleaning) and is prohibited from discharge to any location on Travis AFB without prior approval from the 60 CES/CEAN. The contractor shall obtain written approval from the CO prior to discharging to any Travis AFB sanitary or storm sewer system.

Hazardous Material Storage Tanks. The contractor is responsible for ensuring compliance with 40 CFR 112, the California Health and Safety Code Chapter 6.67, Section 25270.3 and the 60 AMW Integrated Contingency Plan (Latest version) for any above ground storage tank brought to the base for contractor’s use. There are no size exemptions on Travis AFB. Requirements include conducting daily visual inspections, proper labeling, secondary containment, proper grounding, etc. Vehicle/equipment filling and storage tank refilling must be monitored continuously and conducted in a manner to prevent spills and overfills. The Government shall audit their tank area to make sure in compliance with requirements. (N/A)

Spill Response. The contractor shall be responsible for the clean-up and disposal of all spilled materials. This includes all materials use to contain and absorb the spill. Spilled material must be cleaned-up promptly and reported to the CO. If any amount of spilled material contacts or has the potential to contact water, soil or any drain (sanitary or storm), the contractor shall call 911 (424-4911 if utilizing telephone on base) immediately. Spills of HAZMAT or hazardous waste shall be managed in accordance with the current 60 AMW Integrated Contingency Plan for Oil and Hazardous Substances Spill Prevention and Response for Travis AFB. Disposal of wastes generated from spill clean-up shall be the contractor's responsibility. The contractor shall be liable for reimbursement of containment and environmental clean-up of the spills.

Training. The contractor is responsible to train their employees regarding Environmental and Hazardous Materials Handling. The contractor is also required to have and maintain Safety Data Sheets (SDS) for all materials used by the contractor in accordance with but not limited to federal and state laws and/or regulations.

Fire Safety. The contractor shall obtain a burning permit from the Travis AFB Fire Department prior to welding, burning or cutting operations each day they anticipate performing this type of work. All welding and burning operations necessary for the completion of the PWS shall be accomplished in strict compliance with fire safety and applicable standards. The contractor shall provide and maintain fire extinguishers in accordance with NFPA standards during all welding and burning operations relative to the PWS.

Safety. The contractor shall conform to all safety regulations to include but not limited to: Occupational Safety and Health Administration (OSHA) and the Maintenance Manual for each piece of equipment. The contractor is responsible for inbound and outbound traffic control while working on the equipment and assuring proper coordination is made with the security forces office, emergency response office(s) and the COR at least one day prior to scheduled maintenance. The COR will contact the contractor for all emergency work required and coordinate best time to perform this maintenance.

SECURITY REQUIREMENTS: Travis Air Force Base is designated as a closed base. In order to promote security and safety, all employees desiring access must adhere to installation entry requirements, to include, identity proofing and vetting. This includes a National Crime Information Center (NCIC) and California Law Enforcement Telecommunication System (CLETS) check. Identity proofing and vetting is not required for employees if they have a current favorable government security clearance which can be verified through the Joint Personnel Adjudication System (JPAS).

The primary contractor will ensure all employees possess proper credentials allowing them to work in the United States and ensure illegal aliens are not employed and/or transported onto the installation. Identification required for identity proofing is:

Permanent Registration Card/Alien Registration Receipt Card ( Form I-1551)

Foreign Passport with a temporary (I-1551) stamp or temporary (I-1551) printed notation on a machine readable immigrant visa.

Employment authorization document that contains a photograph (Form I-766)

In the case of a nonimmigrant alien authorized to work for a specific employer incident to status, a foreign passport with Form I-94 or Form I-94A bearing the same name as the passport and containing an endorsement of the alien’s nonimmigrant status, as long as the endorsement has not yet expired and the proposed employment is not in conflict with any restrictions or limitations identified on the.

The contractor shall not be entitled to any compensation for delays or expenses associated with complying with the provision of this clause. Furthermore, nothing in this clause shall excuse the contractor from proceeding with the contract as required.

Identity Proofing and Vetting. Employees whose background reveals any of the following disqualifiers will not be allowed installation access. All employees will be vetted based on the following disqualifying base access criteria:

The individual is known to be or reasonably suspected of being a terrorist or belongs to an organization with known terrorism links/support.

The installation is unable to verify the individual's claimed identity.

There is a reasonable basis to believe the individual has submitted fraudulent information concerning his or her identity.

The individual has/had been debarred from entry/access to a Federal installation or facility.

The individual is wanted by Federal or civil law enforcement authorities, regardless of offense or violation.

The individual has any conviction for espionage, sabotage, treason, terrorism, or murder.

The individual’s name appears on any Federal or State agency’s watch list or hit list for criminal behavior or terrorist activity.

Within the past 10 years, the individual has been convicted of a firearms or explosive violation.

The individual has been convicted of sexual assault, armed robbery, rape, child molestation, child pornography, trafficking in humans, drug possession with intent to sell or drug distribution.

The individual has knowingly and willfully engaged in acts or activities designed to overthrow the U.S. Government by force.

There is a reasonable basis to believe the individual will attempt to gain unauthorized access to classified documents, information protected by the Privacy Act, information that is proprietary in nature, or other sensitive or protected information.

There is a reasonable basis to believe, the individual will unlawfully or inappropriately use an access credential outside the workplace.

Within the past 10 years, the individual has received 2 or more convictions regardless of the crime. Thus, there is a reasonable basis to believe based on an individual's criminal or dishonest history, that issuance of an access credential poses an unacceptable risk to the installation/mission.

There is a reasonable basis to believe, based on the individual's material, intentional false statement, deception, or fraud in connection with Federal or contract employment, that issuance of an access credential poses an unacceptable risk to the installation/mission.

Within the past 3 years, the person being vetted has 2 DUI convictions or more. Thus, there is a reasonable basis to believe, based on the nature or duration of the individual's alcohol abuse without evidence of substantial rehabilitation, that issuance of an access credential poses an unacceptable risk to the installation/mission.

Within the past 3 years, the person being vetted has 2 drug possession convictions. Thus, there is a reasonable basis to believe, based on the nature or duration of the individual's illegal use of narcotics, drugs, or other controlled substances without evidence of substantial rehabilitation, that issuance of an access credential poses an unacceptable risk to the installation/mission.

Within the past 10 years, the individual has been incarcerated for a period of 1 year or more unless released on proof of innocence.

There is reasonable basis to believe based on an individual’s criminal or dishonest history, that issuance of an access credential poses an unacceptable risk to the installation/mission.

Employees will be identity proofed and vetted each time a pass is issued. Security Forces may conduct random screenings at any time; if, disqualifying base access information is found employees may have their passes revoked.

Employees with disqualifying base access information will be issued a denial access letter immediately revoking their base access privileges. Employees requesting to contest the adjudication, denial of installation access, or requesting a waiver/exception to policy must submit a written rebuttal/request within 10 business days of receipt of the barment letter to 60 AMW/CC, through 60 SFS/CC, Attention: 60 SFS/S5R, Bldg. 381, 540 Airlift Drive, Suite C- 101, and Travis AFB 94535-2451.

Primary Contractor Responsibilities. The primary contractor will be responsible for the conduct of all employees working under that contract. Additionally, the primary contractor will:

Coordinate base entry requirements with the 60th Contracting Squadron.

Advice employees requesting base access on requirement to complete, sign and submit a Travis Form 251, Consent for Background Check. Forms are available at the Pass and Registration Office and Visitor Control Center. Any employee who does not complete or sign the form will be denied installation access.

Advise employees the base pass is only valid for the purpose, person and vehicle for which it was issued. Use of the base pass for any other purpose or by any other person will result in the pass being confiscated. Employees that misuse their pass may be subject to barment actions. If a pass is lost, notify the Pass and Registration Office immediately.

To obtain a pass, personnel will need a valid state or government photo identification.

To obtain a vehicle pass, personnel will need a driver license, registration and insurance.

Provide written notification, within 24 hrs. To the 60th Contracting Squadron of any changes in employee’s status. This includes, but is not limited to, the employee being fired or quitting their position with the company.

Retrieve government issued personal and vehicle passes from employees which no longer need installation access. Passes will be turned into the 60the Contracting Squadron upon expiration.

Obtaining a Base Pass. Provide an EAL (Entry Authority List) of all employees on company letterhead which require a base pass. All requests for a base pass will be submitted through the Base Contracting Office NLT 45 days prior to the contract start date. Exceptions will be made for short-notice contracts where the award date and performance start date are less than 45 days. A base pass will be issued for a maximum of one year. Prior to renewing a base pass, return the old base pass to the Pass and Registration for destruction. Ensure the EAL includes:

Contract number, Work site or location, Inclusive dates of the contract, Work schedule (include days of the week and time periods employees are on base), Employee’s full name, date of birth, and social security number.

Only persons who have undergone identity proofing and vetting and have no disqualifying base access information can serve as a sponsor. Persons appointed as sponsors will meet employees at the Visitor Control Center and ensure they complete Travis Form 251, Consent for Background Check.

Employee Responsibilities. All employees requiring reoccurring and unescorted access onto the installation must:

Carry their DoD ID card or installation pass on their person while on the installation. Register privately owned vehicles in accordance with installation policies.

On request, present their DoD ID card or installation pass to security personnel. Refusal may be grounds for further administrative or punitive action.

If issued a Command Access Card (CAC), present documentation from the local security office or CAC sponsor confirming that the CAC has been reported lost or stolen.

Turn in access credentials to the 60th Contracting Squadron when the credential expires or when the basis for obtaining the credential no longer exists.

Increased Force Protection Condition (FPCON). During FPCON Normal, Alpha and Bravo; employees without a base issued pass must be sponsored onto the installation. During FPCON Charlie and Delta the base will curtail non-essential operations/functions and non- essential employees will be suspended at the direction of the installation commander. All employees attempting installation access; thereafter, will be physically escorted unless FPCON Mission-Essential designation has been approved in advance and is indicated on the base pass.

Restricted Area Badges. Employees may be submitted for unescorted entry into restricted areas if required for their contract. Contact the security manager for the military agency responsible for the project.

Escort Requirements. The following escort requirements apply:

6.7.1 Installation. Sub-contractors must be escorted at all times.

Controlled/Restricted Areas. Employees not in possession of a restricted area badge will be escorted at all times when working within controlled, restricted or other sensitive areas by a government representative with a restricted area badge. Escorts can be either the military agency responsible for the project or an employee in possession of a restricted area badge. The military agency or employee in possession of a restricted area badge will follow existing procedures and instructions for obtaining entrance to controlled, restricted and sensitive areas.

Lost Base Passes or Restricted Area Badges.

Base Passes. The employee’s supervisor will investigate and provide written notification for a lost base pass to the 60th Contracting Squadron and COR. Written notification should include an explanation from the employee on how, when, where and what steps have been taken to locate the missing base pass. If a replacement is needed, forward the notification with the request for a base pass.

Restricted Area Badges (RAB). Employees issued a RAB must report the loss immediately to the security manager of the military agency that submitted the RAB request.

The individual who lost the RAB will provide a written explanation on how, when, where and what steps have been taken to locate the missing RAB. The security manager will conduct their own inquiry and forward a report of investigation [with squadron commander endorsement]; the member’s written explanation and the original AF FM 2586 to the Pass and Registration office. A new RAB will not be issued until the investigation is complete.

6.10. Identification Common Access Card (CAC). Employees requiring computer access must submit a request through the security manager for the military agency responsible for the project.

The security manager will facilitate registration within the Contractor Verification System. The COR is designated as the sponsor and will assist with this process. Once issued, the CAC card must be in the employees possession at all times while on base and performing official duties. The primary contractor shall advise their employees that a favorable security clearance background investigation is needed as a condition of employment under this contract.

Anti-terrorism Force Protection Training. IAW AFI 10-245 and TAFB OPLAN 10-245, all employees with contracts over 90 days, will complete initial and annual Level I Anti-Terrorism Awareness training at https://atlevel1.dtic.mil/at/ . All personnel will be responsible to provide proof (copy of training certificates) of training to the Anti-terrorism Advisor/Representative responsible for the unit they are contracted.

SPECIAL QUALIFICATIONS. The contract manager and alternate(s) must be able to read, write, speak and understand English.

Restricted Areas. Contractors working in and around the restricted areas and/or controlled areas must receive a Security Awareness Phase I Orientation Training conducted by the installation Security Constable before entering a restricted area.

RECORDS MANAGEMENT: Ensure that all records created as a result of processes prescribed are maintained in accordance with AFMAN 33-363, management of records, and disposed of IAW the Air Force Records Disposition Schedule (RDS) located at https://www.my.af.mil/afrims/afrims/arfims/rims.cfm.

GOVERNMENT FURNISHED/PROPERTY/UTILITIES/MEDICAL.

Property. The COR shall provide a location to the Contractor for the sole purpose of storing test weights required during the load tests of the cranes and hoists specified in this PWS. The Contractor shall be required to secure and safeguard the Contractor’s test weights when using these designated areas, as the Government is not responsible for theft, damage or loss. The location for storing these weights will be in the parking lot of building 1205 South side and the COR will coordinate this with the contractor.

Utilities. The Contractor shall be provided utilities by the government, i.e., electricity, water, restrooms, as needed in performing the work in the PWS. The Contractor shall conserve utilities provided.

Medical Services. The Contractor and their employees are authorized to use the David Grant Medical Center (DGMC), located at Travis Air Force Base, CA for emergency situations only. All incurred costs for providing any or all services shall be reimbursed to the Government.

APPENDIX A

Workload Estimates and Data The table provides the location, quantity and other pertinent information of the lift equipment. Each Crane/Hoist below requires 3 Quarterly Inspections and 1 Annual Load Test and Inspection.
