Attachment 1 Appendix 7 Reporting Requirements

Reporting Requirements
Description
1) Monthly Status Report
The report will include at a minimum the information required for each G-391 for every trip as indicated in the G-391 Upload Template attachment (see attached Excel file). A breakdown of hours and personnel will also be provided and divided into Transportation Guard Hours and Stationary Guard Hours. A breakdown of vehicles used (year, model, and capacity) will also be required if the contractor is using contractor owned vehicles. This information will be available electronically to government users and submitted in addition to the invoice each month.
2) Quarterly Performance Report
This report will be produced every three months to document and provide the transportation data collected from all movement of ERO serviced contract hours for the previous quarter. It will include a summation of the previous Monthly Status reports and document any fluctuations in demand or trends in provided service. Recommendations for surges or lulls will also be included in the quarterly performance report along with the vendor’s capability to respond.

Sample G-391 Data Categories

Data categories required for every transportation movement:

Description
1) Date
Date of the movement.
2) Vehicle Owner/Number
The assigned vehicle owner (Contractor/ICE) and fleet number for the vehicle.
3) Vehicle Type
The type of vehicle used during that movement (e.g. van, bus, sedan).
4) Start/End Location
The location from where and to where aliens are transported.
5) Start/End Mileage
The mileage reading at the start and end location.
6) Start/End Time
The time at the departure and arrival of the movement.
7) # of Contract Officers
The number of contract officers moving aliens, including their names.
8) # of ICE Officers
The number of ICE Officers moving aliens, including their names.
9) # of Aliens Departing
The number of aliens entering a vehicle at a location.
10) # of Aliens Delivered
The number of aliens exiting a vehicle at a location.
11) Alien Description
Gender, Juvenile, Family Unit, etc. that were transported.
12) Movement Types
Select from list of movement types below
a. Transfers
Transporting of aliens in custody from one ICE location to another.
b. Legal
Transporting of aliens for legal appointments (i.e. court, lawyer or consulate visits).
c. Medical
Transporting detainees to a hospital or clinic for medical reasons.
d. Ambulatory
Chasing an ambulance
e. Air Transfers
Transporting aliens in custody to or from an airport for domestic transfers.
f. Air Removals
Ground transportation of aliens to an airport for final removal via air.
g. Land Removals
Ground transportation of aliens to their country of origin for final removal. (e.g. busing or walking aliens into Mexico).
h. Pick up
Apprehension of an alien from a non-ICE location. (e.g. pick up from Jail / Prison).
i. Release
Transporting aliens that have been released from custody to a U.S. domestic location (i.e. bus terminal).
j. Stationary Duty
Any duty requiring the supervision of aliens by a guard staff that does not require a movement by vehicle.
k. Other
Transportation for a reason other than moving aliens (e.g. vehicle maintenance, file transfers).
13) General Comments
Use this space as a general comments field. Document any significant events, issues, instructions, etc. related to the movement.
