About this file
This is a pricing table template for a labor-hour based contract for Non-medical Use (NMU) Data Report Services with the FDA. Offerors must complete the table by providing loaded labor hour rates for seven specified labor categories: Project Manager, Senior Computer Systems Analyst, Statistician, Pharmacist, Epidemiologist, Survey Scientist, and Subject Matter Expert. The contract spans five ordering periods with pre-established labor hour quantities that cannot be modified.
The pricing table is structured around three primary activities: (1) Quarterly Teleconferences and Kick-Off/Closeout Meetings, (2) Task Order Reports, and (3) Contingency Labor Rates for impromptu communications and collaborations. Ordering Periods 1-4 each include four quarterly teleconferences and eight task order reports across all labor categories, while Ordering Period 5 includes three quarterly teleconferences and eight task order reports, concluding with a closeout meeting. The contingency labor rates remain consistent at 40 hours per labor category across all periods for unscheduled work. Offerors must not alter any pre-set quantities, add labor categories, or leave any pricing fields blank; failure to comply will result in proposal rejection without further consideration. Subtotals are required for each ordering period, with a total evaluated price calculated at the end.

| Attachment A Pricing Table Please complete the following pricing table and return it with your proposal. As stated in the RFP, the quantities of labor hours provided are not guaranteed quantities. Rather, the actual quantities will be determined as Agency needs arise during contract performance. Nonetheless, do not adjust the pre-set quantities in the pricing table. Also, do not add labor categories or activities to the pricing table. If the offeror changes any of the pre-set quantities in the pricing table, fails to provide any per hour pricing, otherwise fails to provide a total price, or adds labor categories or activities to the pricing table, the offeror’s proposal shall be discarded as unacceptable and given no further consideration. |
