PERFORMANCE WORK STATEMENT

PHYSICAL EXAMINATION

TESTING FOR

POST OFFER EMPLOYMENT NEW HIRE

CANDIDATES AT

DLA DISTRIBUTION WARNER ROBINS, GEORGIA

1. BACKGROUND

The Defense Logistics Agency (DLA) is a United States (US) Department of Defense (DOD) agency that provides worldwide logistics support for the missions of the Military Departments and the Unified Combatant Commands under conditions of peace and war.

DLA Distribution Warner Robins, Georgia is located at 455 Byron Street, Robins Air Force Base, Georgia 31098. Their mission is to provide a full range of distribution services using a seamless, tailored, worldwide Department of Defense distribution network that delivers effective, efficient and innovative support to combatant commands, military services, and other agencies during peace and war.

The nature of the work performed at the depot require employees to stand on hard surfaces for extended periods of time. Employees continuously bend, stoop, and work in tiring and uncomfortable positions. Employees are expected to lift and carry items weighing up to 40 pounds which may be bulky and stored on pallets or in high-level locations (up to 40 feet).

They may use weight handling equipment or receive assistance from other workers for items heavier than 40 pounds. Moderate physical effort is used in operating hand and foot controls when operating material handling equipment.

Working conditions include working outside and inside in areas that are hot, damp, cold, drafty, and poorly lit. Employees must use protective clothing such as steel toed shoes, respirators, safety glasses, safety harnesses, and earplugs. Mechanized roller conveyors are often present as a normal part of material routing and distribution. Employees must possess or be able to obtain and maintain all appropriate state and government licenses for all autonomous weight handling equipment. Working conditions are a normal part of the job and are considered in the evaluation of the position.

DLA Distribution Warner Robins, Georgia requires pre-employment physicals (health screenings). These pre-employment physicals provide a medical baseline.

2. OBJECTIVE

DLA Distribution Warner Robins, Georgia seeks to obtain new hire candidates who possess a high probability for job success and a low probability for job injury.

For DLA Distribution Warner Robins, Georgia, health and vital screenings are fundamental to achieving this objective. Pre-employment physicals will provide a baseline to serve towards this objective.

3. SCOPE OF WORK

DLA Distribution Warner Robins, Georgia has a requirement for a Contractor to perform physical examination testing for new hire candidates at DLA Distribution Warner Robins, Georgia, as set forth in this PWS.

All references in this PWS to “Contractor Personnel” include both Contractor employees and subcontractors.

This is a non-personal services contract to provide physical examination testing for post offer employment new hire candidates at DLA Distribution Warner Robins, Georgia. The Government will not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. The Government and the Contractor understand that the services to be provided under this contract by the Contractor are non-personal services and that no employer-employee relationship exists between the Government and the Contractor. The Government may provide technical direction to assist the Contractor in accomplishing the PWS; however, the Government will not control the methods used by the Contractor to perform the service requirements set forth in the PWS.

The contractor shall provide all labor, material, equipment and supervision necessary to perform pre-employment physicals complete and in accordance with this PWS. These tests will include, but may not be limited to CBC, CMP, Lipids, Audiograms, Vision, Vitals, EKG (With Interpretation), and HIV Testing.

The Government will provide the Contractor with Optional Form (OF) 178 – Certificate of Medical Examination, or its equivalent/replacement at time of award. This form will identify the purpose of the medical examination; describe the position description along with standard requirements; and outline the specific functional requirements and environmental factors that the work requires for each candidate requiring testing.

4. PLACE OF PERFORMANCE

The Contractor shall perform physical examination testing for new hire candidates at his designated facility. For DLA Distribution Warner Robins, Georgia’s new hire candidates, the designated facility shall be located within a 40 mile one-way range from DLA Distribution Warner Robins, Georgia.

More than one facility located within the 40 mile driving distance may be designated. The Contractor shall obtain approval from the applicable Government Point of Contact (POC) for changes to designated testing facility locations.

5. PERIOD OF PERFORMANCE

Refer to solicitation/contract for applicable period of performance.

6. SPECIFIC REQUIREMENTS

The Contractor shall provide all personnel, facilities, equipment, materials, supplies and services necessary to develop, perform and manage the physical examination testing for DLA Distribution Warner Robins, Georgia.

6.1 PHYSICAL EXAMINATIONS

The Government Point of Contact (POC) will provide a list of new hire candidates to the Contractor. Within 3 business days after receipt of the list, the Contractor shall contact each new hire candidate to schedule the physical examination. The physical examination shall be scheduled within 5 business days after initial contact with the new hire candidate. Physical examinations shall include all tests as indicated on the Office of Personnel Management (OPM) Optional Form 178 provided for each individual new hire candidate based on position description. A sample Optional Form 178 and position description for the Distribution Process Worker position is attached to the PWS as an example only. Within 3 business days after candidate’s completion of the physical examination, the Contractor shall forward the completed Optional Form 178 in accordance with the instructions contained therein.

6.2 APPOINTMENT CANCELLATIONS

The Government Point of Contact (POC) or the new hire candidate may cancel the scheduled physical examination testing within 3 business days prior to the appointment date with no penalty or charges assessed to the Government. If the Contractor is not notified of a cancellation within 3 business days prior to the scheduled appointment date, the Contractor may invoice for the missed appointment at a rate not to exceed 50% of the normal per candidate testing/evaluation rate established in the Schedule of Supplies/Services. If the Contractor needs to reschedule a physical examination testing appointment, the Contractor shall notify the new hire candidate and the Government Point of Contact (POC) within 3 business days prior to the appointment date and shall establish a new appointment date. The Government will not incur any charges associated with Contractor rescheduling.

7. PERSONNEL QUALIFICATIONS AND CERTIFICATIONS

The clinician shall have and maintain a current medical license in good standing with the State of Georgia and have a minimum of 3 years work experience in occupational medicine. The clinician shall conduct an in depth review of vital signs in a clinical setting to perform pre-employment testing on a case by case basis as directed on an Optional Form (OF) 178.

The Clinician shall possess a working knowledge of Federal Occupational Safety and Health Administration (OSHA) Code of Federal Regulations (CFR), be able to evaluate and interpret test results, and possess experience in conducting conditional pre-employment assessments and making specific oral/written recommendations based on test outcomes.

The clinician should possess knowledge of medical requirements/standards associated with specific position descriptions and have the ability to assess Government personnel for employment in accordance with DOD standards of fitness.

8. SAFEGUARDING INFORMATION

The Contractor and his personnel shall not divulge or release data or information developed or obtained under performance of this contract except to government personnel who are authorized to receive information or upon written approval of the Contracting Officer (KO).

9. MANDATORY REPORT – CONTRACTOR MANPOWER REPORTING

The Contractor shall report all Contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Defense Logistics Agency via a secure data collection site. The Contractor is required to completely fill in all required data fields using the following web address:

http://www.ecmra.mil. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2013. 10. CERTIFICATION AND ACCEPTANCE

The Government Point of Contact (POC) will monitor performance to ensure services are received and carried out in accordance with the requirements set forth in this PWS. This monitoring shall be done via Government inspections and Government review of applicable documentation. The Government Point of Contact (POC) is designated as the point of final inspection and acceptance by the Government for services required by the contract.
