Performance Work Statement (PWS)

For

JLG Preventative Maintenance

1. The primary objective of this Performance Work Statement (PWS) is to provide an outline of the preventive maintenance required for JLG aerial lifts in accordance with the manufacturers recommended preventive maintenance and standard commercial practices for the following pieces of equipment.

1.1. Equipment to be serviced

1.1.1. Bldg. 282 - Structural Maintenance Corrosion Section

1.1.1.1. JLG Lift, Model 660SJ – Serial # 03000102777

1.1.1.2. JLG Lift, Model E450A – Serial # 0300134023

1.1.2. Bldg. 233 – Wash Rack

1.1.2.1. JLG Lift, Model 45E – Serial # 0300039499

1.1.2.2. JLG Lift, Model 45E – Serial # 0300023241

1.1.3. Bldg. 250 – ISO Maintenance Section

1.1.3.1. JLG Lift, Model E450AJ – Serial # 0300071546

1.2. The contractor shall provide supervision, personnel, maintenance equipment, transportation, tools, and labor necessary to inspect and maintain the equipment noted above as prescribed in this PWS. The contractor will provide all required safety equipment. All equipment used under this contract shall meet all OSHA, EPA, and any other applicable guidelines. All safety equipment must be used for its intended and designed purpose. Unsafe acts by the contractor will be reported to contractor supervision and 19 CONS and may be grounds for contract termination. The U.S.

Government is not responsible for any travel costs, per diem, mileage, or lodging for this contract.

1.2.1. The contractor will not use any government equipment or personnel in the performance of their duties unless section supervision determines it is required for the completion of this PWS.

1.3. The contractor shall provide a descriptive and comprehensive inspection checklist and maintenance schedule for above pieces of equipment for the U.S Government to check on a daily and weekly basis. The inspection criteria will be based on manufactures servicing manual recommendations for inspection frequencies of manuals listed in paragraphs 3.2-3.4.1. Any deficiencies found during these inspections by the U.S Government will be handled on a case by case basis depending on severity in accordance with contractor recommendations and the procedures defined in this PWS.

1.4. Contractor preventive maintenance will be performed on a quarterly basis as per this PWS, unless otherwise written. Some maintenance items may be performed on a quarterly, semi-annual or annual basis for on the above equipment items under the terms in paragraph 3.0.

1.5 The contractor shall provide all the necessary non-consumable parts required for preventative maintenance of the equipment in accordance with the applicable items service manual. The contractor shall obtain written permission from the Contracting

Officer prior to ordering any parts $500 or more. Approval of parts and labor will be required prior to maintenance being performed. Also, suitable substitute parts that do not meet original manufacture specifications shall be approved by the Contracting Officer prior to purchase and installation. A written cost estimate of parts and labor will be delivered to the Government Representative and Contracting Officer no later than 72 hours after discovery of need of item or part.

1.6 For the purpose of this PWS, preventative maintenance is defined as those tasks stated and/or recommended by the specific equipment manufacturer and the contractor to keep the machine/equipment in safe, working order, with the goal to further the equipment’s service life. In the absence of a manufacturer preventive maintenance directive, the contractor will use standard commercial preventive maintenance best practices.

1.7 The contactor will provide at no expense to the government the repair or replacement of components and equipment due to abuse, misuse, mishandling or neglected maintenance of the contractor or by those employed by the contractor.

1.8 If equipment is down for maintenance for an extended period due to parts back order, awaiting maintenance status for technician repairs, or building maintenance during the scheduled preventive maintenance, the preventive maintenance will be completed on that item at another scheduled time approved by the Contracting Officer. The U.S Government will coordinate the preventive maintenance site visits with the contractor when equipment servicing in required. There will be no costs to the government if no preventive maintenance is accomplished during that period. If an item is unserviceable during a preventive maintenance cycle and only becomes serviceable during the next scheduled cycle, the government will only have to pay the costs of one preventive maintenance cycle for work accomplished.

1.9 All unnecessary or discarded parts will be removed immediately from the job site by the contractor and will be disposed of at no cost to the government. The use of dumpsters on Little Rock AFB by the contractor to dispose of hazardous waste is prohibited. Disposal of regular trash in government refuse containers shall be reviewed and approved by the Government Representative.

1.9.1. Cardboard boxes or parts packaging shall be broken down and disposed of in applicable recycling containers by the government. The government will dispose of the packaging as necessary.

2. Basic Services – The contractor shall provide the basic services as described in this PWS.

2.1. Inspection Checklist - The contractor will provide this checklist of the items that will be inspected no later than 30 days after the first maintenance visit. This will be a listing of all actions performed during the visit. The checklist will be broken down by the section responsible, building and piece of equipment. This checklist may be in an editable digital form (such as Word or Excel format). This will be a concise listing of preventative maintenance done by the contractor in accordance with this PWS during the time frames for all stated equipment listed in paragraph 1.0.

2.2. Quarterly Preventative Maintenance Scheduling - The contractor will contact the Government Representative no later than one week prior to scheduling quarterly preventive maintenance. The contractor will contact the Government Representative at 501-987-2951 to schedule appointment. If contractor cannot a Government Representative, contact the Resource Advisor at 501-987-3225. If the contractor cannot keep an appointment, the appointment must be rescheduled no later than 24 hours prior to appointment time and rescheduled no later than 24 hours later of the previously agreed upon date.

2.3. Contractor Check-in - Prior to the start of any preventive maintenance, part installation, parts deliveries, or site inspections, the contractor must check in with a Government Representative where maintenance is being performed. Due to USAF mission requirements and needs of the government, there may be a delay for supervision availability. No work will be accomplished until checked in with a Government Representative.

2.4. Contractor Actions at Worksite - Contractor technicians will maintain control of all contractor tools and equipment at the jobsite. At no time should the government be responsible for lost, damaged, or missing tools, equipment, or parts that are property of the contractor on the jobsite. Lost tools and equipment will immediately be brought to the attention of a Government Representative. Any debris or trash created by the contractor or replaced parts on the jobsite will be disposed of in accordance with paragraph 2.5.1 of this PWS. Contractor personnel are only to be in their assigned areas and are not allowed in other secure aircraft maintenance areas without Government Representative approval and/or escort.

2.5. Contractor Checkout - After finishing any preventive maintenance, part installations, part deliveries, or site inspections, the contractor must check out a Government Representative whom will inspect all work performed and will ensure work has been accomplished in accordance with (IAW) this PWS and meets requirements necessary for the government. If the Government Representative cannot be reached, then call 501-987-3225 to reach the Squadron Resource Advisor and they will provide personnel to inspect the site.

2.5.1. Any dust, dirt, debris on floor that is caused by the contractor will NOT be swept up in building 282 as there are specific guidelines for Corrosion Facility Housekeeping. Building 282 personnel will clean these areas as required.

However, said items may be swept up in buildings 233 and 250. Items must be cleaned before the contractor leaves and will be disposed of IAW this PWS.

Unless otherwise stated in this PWS, the contractor will remove all debris from area and will not dispose of items in the section’s trash or trash dumpsters unless approved by a Government Representative. The areas will be checked at contractor checkout and the contractor will not be released until the area is clean.

2.5.3. At this time, a receipt for work accomplished will be given to a Government Representative as proof work was performed. This receipt will also be sent to the 19 Maintenance Squadron (MXS) resource advisor (RA) and invoiced through the proper channels. The contractor is still required to invoice through Wide Area Workflow (WAWF). This receipt will assist the squadron RA to sign off WAWF invoices quickly.

2.5.4. Any work accomplished that is not according to this PWS or items that require rework due to workmanship or neglect will be up to the Contracting Officer to have rework accomplished on the spot or to schedule an appointment for a later time. This rework will be documented and sent to the squadron RA and will be elevated to contractor management. There will be no additional costs to the government as any related costs will be the contractor’s responsibility.

2.6. Unscheduled Maintenance - Equipment items that require repairs or maintenance done that are not considered preventive maintenance, are considered unscheduled maintenance. These unscheduled service calls include the labor to troubleshoot and installation of parts. See below in paragraph 2.7 for types of service calls. Repairs will be billed to this contract only after review and approval from the Contracting Officer and will be processed through WAWF. If no parts are required, there will be no additional costs to the government besides invoicing the service call.

2.7. Service Calls - In addition to preventative maintenance at set times, the contractor shall provide support of unscheduled maintenance troubleshooting and repairs. A service call is required for troubleshooting, adjustments, and failed part(s) replacements. The contractor will provide support (labor) for normal unscheduled service. Service calls for warranty work from previous service calls from the contractor will not be counted toward the section’s service calls. Each type of service call is broken down in paragraph 2.7.1 – 2.7.2.

2.7.1. Items Covered - The contractor shall be able to provide support for unscheduled maintenance service calls for all JLG’s as listed in paragraphs 1.1 -1.1.3 of this PWS. Parts costs are separate and will be handled IAW this PWS.

All final repairs will have a 1-year warranty and any rework is the responsibility of the contractor.

2.7.2. Normal Unscheduled Maintenance Services - The contractor will make provisions to provide service calls for unscheduled maintenance. The contractor will be able to have technicians on site within five (5) business days.

The contractor shall utilize the set aside amount and adhere to the procedures and restrictions in this PWS for any parts that are required. Any travel and lodging costs are the sole responsibility of the contractor.

2.8. Parts Not Listed as Consumable - The contractor must be able to supply the U.S Government with all the necessary consumable and non-consumable parts as required for maintenance of the aforementioned equipment. The contractor will provide a cost estimate in writing of all parts and or services not covered by this PWS to the U.S Government prior to work approval. After work is approved, the contractor will submit all charges through WAWF for payment against this contract.

2.8.1. Non-Consumable Parts - The Government Representative will contact the contractor for trouble shooting via a service call at no additional cost, and the item will be replaced either at the next scheduled preventative maintenance if there is less than 30 days until the next call if the equipment is still operational. If the equipment is not operational, the Contracting Officer may request a service call defined in paragraph 2.7. If the part is back-ordered, the contractor will contact the Contracting Officer every 7 days to provide a status update. If part does not arrive within 30 days, the Contracting Officer has the option to have another contractor order and install the part and the contractor will not be able to invoice the service call.

2.8.2. Parts - Any charges for parts required to perform this contract shall utilize CLIN X002 and X004. The contractor shall not order any parts if the resulting costs exceed the remaining balance of the appropriate CLIN. In the event parts are required, and the required funds are not available, the Government Representative shall contact the Contracting Officer in order to determine if a contract modification, separate purchase, or other procurement method shall be pursued.

Preventative Maintenance- CLIN X001 shall be utilized for any charges relating to servicing (i.e. labor) for preventative maintenance costs that occur as part of this contract. As noted in paragraph 2.8.2, no work shall be performed in the costs exceed the remaining balance of CLIN X001. Any travel and lodging costs are the sole responsibility of the contractor.

3. Specific Preventive Maintenance Actions for Equipment - Preventive maintenance will be done on a quarterly basis (unless otherwise stated in this PWS) by the contractor for the equipment items listed in paragraph 1.0.

3.1. General Guidance

3.1.1. Contractor will provide the government with a checklist of preventative maintenance actions that will be performed each quarter and will give a copy of the checklist of items to be accomplished during the monthly preventative maintenance visit.

3.1.2. Once the contractor has completed the quarterly preventative maintenance, the contractor will provide the Government Representative with the checklist showing everything that was accomplished during the preventative maintenance within 3 days of completed maintenance.

3.1.3. If parts are found to be needed for next quarter’s preventative maintenance, the contractor will give a list of items that will be needed and the Government Representative or Contracting Officer (CO) will approve ordering the parts. This list is to be given to the shop within 3 days of completing maintenance.

3.1.4. If the checklist and/or list cannot be provided by the contractor within the allotted 3 days due to unforeseen circumstances, the contractor will contact the Government Representative within 3 days. The Government Representative and contractor will determine an applicable timeframe.

3.2. Paint Hangar, Building 282. JLG Lift, Model 660SJ-Serial # 03000102777, and JLG Lift, Model E450A-Serial # 0300134023

3.2.1. The contractor will be responsible for all listed JLG equipment servicing, maintenance, and inspection related items associated with the paint hangar, building 282 in accordance with Attachment 1 and Attachment 2 (Or subsequent manual revisions).

3.3. Wash Rack Building 233: JLG Lift, Model 45E-Serial # 0300039499, and JLG Lift, Model 45E-Serial # 0300023241

3.3.1. The contractor will be responsible for all listed JLG equipment servicing, maintenance, and inspection related items associated with the wash rack hangar, building 233 in accordance with Attachment 3 (Or subsequent manual revisions)

3.4. ISO Maintenance Section, Building 250-cube 2. JLG Lift, Model E450AJ-Serial # 0300071546

3.4.1. The contractor will be responsible for all JLG equipment servicing, maintenance, and inspection related items associated with the ISO maintenance section hangar, building 250, cube 2 in accordance with Attachment 2 or 4 as applicable to the model (Or subsequent manual revisions)

3.5. Preventive maintenance includes final operational checks of all equipment items.

4. Consumable and Non-Consumable Parts - All equipment will require both parts changed for preventative maintenance. Consumable parts are defined as parts that are used up during the operation of the equipment. Consumable parts include filters, seals, and greasing or fluids. Non-consumable parts are defined as parts that need changed due to the failure of the part on an unscheduled basis.

4.1.Consumable Parts - Consumable parts are defined in this PWS as parts that have a limited service life that deteriorate under normal use. All consumable parts needed to fulfill preventive maintenance and tasks outlined in the attached PWS will be provided by the contractor. For this PWS and quote purposes, all consumable parts are included in the quarterly preventative maintenance.

5. Invoicing - Parts ordered for preventative maintenance or repair that arrive at the owning JLG Government Representative shipping address (or work center) may be invoiced once Government Representative and contractor have confirmed arrival. Only after work has been finished can labor or service calls be invoiced.

5.1.Each equipment item and service must be itemized and invoiced separately.

6. 6.0. General Information

6.1.Recognized Holidays: The contractor will NOT be required to perform service on holidays including but not limited to:

6.1.1. New Year's Day- 1 January

6.1.2. Birthday of Martin Luther King Jr.- 3rd Monday in January

6.1.3. President's Day- 3rd Monday in February

6.1.4. Memorial Day- Last Monday in May

6.1.5. Independence Day- 4 July

6.1.6. Labor Day- 1st Monday in September

6.1.7. Columbus Day- 2nd Monday in October

6.1.8. Veteran's Day-11 November

6.1.9. Thanksgiving Day-4th Thursday in November

6.1.10. Christmas Day- 25 December

If these holidays fall on Saturday, the preceding Friday shall be observed. If these holidays fall on Sunday, the following Monday shall be observed. If a holiday falls on a scheduled service day, the Contractor shall be responsible for rescheduling services for the first day following the holiday observance.

6.2.Hours of Operation: The contractor is responsible for conducting business with the individual purchased maintenance plans, except when government facility is closed due to local or national emergencies, administrative closing, or similar government directed facility closing. The contractor shall not be reimbursed when the government facility is closed for the above reasons

6.3.Contractor Personnel: The Contractor shall not employ persons on this contract if such employee is identified to the Contractor as a potential threat to the health, safety, security and general well-being or operational mission of the installation and its population. The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest, nor shall the Contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to the DoD 5500.7-R Joint Ethics Regulations (JER). In addition, the Contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies contained in AFI 64-106, Air Force Industrial Labor Relations Activities. The Contractor shall be prohibited from employing off-duty COR’s who are surveilling any con-tracts/subcontracts awarded to the Contractor. All contract employees depending on particular condition, occasion or place, or job assignment shall require professional, suitable attire. All Contractor employees shall conduct themselves in a professional, courteous manner. The selection, assignment, reassignment, transfer, supervision, management, and control of Contractor employees in performance of this PWS shall be the responsibility and prerogative of the contractor; however, the Contractor shall comply with the requirements set forth in this PWS and Little Rock AFB regulations concerning conduct of employees as referenced herein. The Contractor shall employ and maintain a technically trained and experienced work force

6.4.Worker's Health and Safety: The contractor shall comply with all applicable Federal, State and local legal requirements regarding workers' health and safety. The contractor is solely responsible for determining the legal and safety requirements that apply to his/her activities. These include but are not limited to all Occupational Safety and Health Administration (OSHA) safety rules and regulations, arc flash training, personal protection training and equipment to protect themselves and others from unsafe work conditions. The contractor shall report any accidents or OSHA violations immediately to the appropriate authorities and within 24 hours to the PCO.

6.5.Security Requirements: The contractor shall comply with all security controls directed by the Government Site POC (i.e. Facility Manager, Government Representative).and or Program Manager.

6.6.Government Property Damage: The contractor shall record and report, within 24 hours, to the PCCIE Program Manager, PCO and designated government site POC, all available facts relating to each instance of accidental damage to government property or injury to either contractor or government personnel. The contractor shall not perform any work on the damaged equipment/property until released by an authorized government representative. If the government elects to conduct an investigation of the accident, the contractor shall cooperate fully and assist the government personnel until the investigation is completed.

6.7.Damage to Property or Equipment: The Contractor, at their own expense, shall repair any damage to Government property or equipment that can be attributed to the Contractor. Said repairs shall be made by a skilled workman, and to the satisfaction or the Contracting Officer or a designated representative.

6.8.Emergency Procedures: Contractor employees shall follow the direction of Government employees in regards to emergency evacuation procedures, i.e. fire, tornado, active shooter and bomb threats. It is the Contractors’ responsibility to account for their employees following an emergency evacuation. Contractor personnel shall participate with government personnel in all fire and tornado drills, and at no additional cost to the government

6.9.Contractor personnel and vehicles must possess appropriate passes. These shall be obtained from Installation Security Forces Pass and ID at the Vehicle Inspection Center

6.10. The Contractor is subject to all installation fire, safety, security, and traffic regulations.

6.11. The Government contracts on the understanding that the Contractor is complying with applicable laws concerning workplace safety.

6.12. The Contractor is subject to all citations and penalties for violation of applicable workplace safety laws. The burden of compliance rests with the Contractor.

Enforcement of workplace safety laws is the responsibility of the agency specified in the applicable law.

6.13. The Contracting Officer may notify the Contractor of hazards in Contractor operations that endanger Government personnel or property and require appropriate action. This provision does not constitute assumption of liability for Occupational Safety and Health compliance on the part of the Government or negate other agency responsibility for enforcement of applicable laws.

6.14. Accident prevention principles must be thoroughly integrated into all operations.

Unsafe conditions and practices, as identified by the designated representative, must be corrected immediately.
