PERFORMANCE WORK STATEMENT (PWS)
FOR
WATER WELL #3502 REPAIR
Melrose Air Force Range, New Mexico
Date: 17 Dec 2024
Contract: Pending Repair Well 3502 Attachment 1 Date: Dec 2024
1. DESCRIPTION OF SERVICES. The contractor shall provide all labor, personnel, equipment, tools, materials, vehicles, supervision, qualifications and other items and services necessary to restore Well #3502 to full and safe operations at Melrose Air Force Range (MAFR), New Mexico. The contractor shall perform the requirements of this PWS in compliance with all federal, state, and local laws and industry standards.
1.1 Well #3502. The contractor shall remove old inoperable well pump, bail the well column and install a new 20 Horsepower (HP) submersible well pump and attach it to existing high density polyethylene (HDPE) piping to predetermined optimal depth. The well casing for well #3502 is approximately 400 feet in depth. Once the new well pump is installed, the contractor shall conduct a well test in accordance with (IAW) industry well testing and developmental standards. Existing well specifications: Pump Model - Peerless Submersible 4 Stage 8lb 20 horsepower (HP) 4" Franklin Motor, 3 phase, 480 volts, continuous duty rating
– Gallons per Minute (GPM): 300; Well Depth: 400 Feet; Diameter: 4" X 387'; Column Pipe:
16" X 360’; Head: 400 Feet.
1.2. Disinfection. The contractor shall disinfect the well with Chlorine IAW industry standards.
After 24 hours, 27th Special Operations Medical Group Bioenvironmental Engineering (BEE) Flight will conduct bacteriological tests before the well is considered serviceable. The results of this test will be provided to the Contracting Officer Representative (COR). The COR will provide test results to the contractor, to include when the well is considered serviceable.
1.2.1 The contractor shall ensure that the well head has a proper sanitary seal to include base color scheme painting (Sherwin Williams SW6061, Tanbark) when all testing has been completed prior to the final bacteriological test conducted by the base’s Bioenvironmental Flight.
1.3. Documentation. The contractor shall be responsible for creating and maintaining all pertinent well development reports that are specifically required by The New Mexico Office of the State Engineer. This is to include but not limited to all well pump data, casing depths, pump setting depth, water levels static and residual and GPM well yield. Well specific documentation (pump set depth, GPM) and records shall be turned over to the Contracting Officer (CO) and COR upon completion of all work no later than 14 calendar days or when requested by the CO or
COR.
1.4. Utilities. The Government will allow the use of electricity and potable water (as necessary) for accomplishment of service IAW this PWS.
2. GENERAL INFORMATION.
2.1. Personnel. Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. Employees must be clearly able to read, write, speak, and understand English.
2.2. MAFR Access. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees. The Pass and Identification Section of 27th Special Operations Security Forces Squadron (27 SOSFS) will provide base entry authorizations/passes to each authorized contractor employee. In addition, contractor personnel wishing to operate a privately-owned vehicle on MAFR must comply with all applicable state and base requirements. Entry authorizations must be renewed as required by the 27 SOSFS Pass and Identification Section. All contractor base pass requests must be sent to the point of contact on the contract from 27th Special Operations Contracting Squadron (27 SOCONS) within 3 business days of the time needed for the pass.
2.3. Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes, from employees who depart for any reason before the contract expires (e.g., terminated for cause, retirement, etc.).
2.4. Vehicles. The contractor shall provide and maintain all vehicles necessary to perform the requirements of this contract. Contractor vehicles shall have the company name prominently displayed on both sides of the vehicles. Vehicles shall be clean and always have a neat, professional appearance. All vehicles shall be in operable condition. Vehicle repairs, other than emergency repairs, shall not be accomplished on MAFR. Equipment failure shall not alleviate the contractor from performing any requirement contained in this contract.
2.4.1. Only qualified operators shall be permitted to operate the vehicles or equipment required by this PWS. Vehicle/equipment operators shall possess a valid driver’s license.
2.4.2. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their Contractor-owned vehicles or privately-owned vehicle while on MAFR.
2.4.3. Traffic Laws. Contractor employees shall comply with all base traffic regulations. The contractor is not permitted to use cell phones on MAFR while operating a moving vehicle, unless the device is hands-free.
2.5. Work Hours. The work hours and workdays for the period of performance are 0700 - 1600, Monday through Friday, with the exception of U.S. Federal holidays (listed below).
Additionally, if the contractor needs to work outside of the normal work hours, a request must be submitted to the CO for approval.

2.6. Security Forces and Fire Protection Services. On MAFR: Contact the appropriate agency in case of emergency by dialing 911 from a range or personnel telephone. If dialing 911 from a cell phone, the call may be automatically routed to an off-base agency, such as the city of Clovis, NM. Be clear about whom you are communicating with and what your situation requires.
2.7. Environmental Management System (EMS). All contractor personnel shall complete EMS Awareness training prior to commencement of any activities. Training is available through the learning management system called The Environmental Awareness Course Hub (TEACH) at https://usaf.learningbuilder.com/account/login/?ReturnUrl=%2f. The contractor shall submit the Awareness Training Certificates of Completion to the COR prior to any on-base performance under this contract. Use of Microsoft Edge, Google Chrome or Mozilla Firefox may be required to print certificates.
2.8. Green Procurement Program (GPP). In performance of this contract, the contractor shall use Environmental Protection Agency (EPA) designated recycled content products, Information Technology (IT) Energy Star products/appliances, Federal Energy Management Program (FEMP) Designated Energy Efficient Low Stand By Power products/appliances, U.S.
Department of Agriculture (USDA) Bio based/Bio preferred products, Environmentally preferable products, Electronic Product Environmental Assessment Tool (EPEAT) registered products, Water Sense or other water efficient products, non- or Low Ozone depleting substances under the Significant New Alternatives Policy (SNAP), non or Low toxic or hazardous constituents (e.g. non-VOC paint) and any other environmentally sustainable product/method, to the greatest extent possible.
2.9. Hazardous Materials Handling. The contractor shall comply with all Federal, State and Local regulations (including Cannon Air Force Base & Melrose Air Force Range Civilian Contractor Environmental Guide) concerning the use, storage, and reporting of Hazardous Materials (HM). In accordance with AFI 32-7086, Hazardous Materials Management, the contractor shall provide the following information to the CO for all HM to be used:
(1) Product identification or label
(2) Safety Data Sheet (SDS)
(3) Estimate of quantity to be used
(4) Maximum quantity to be stored
(5) Storage location
2.9.1. The above information must be submitted to the CO on a Cannon AFB Contractor Hazmat Worksheet and Cannon AFB Contractor HM Usage Worksheet (to be provided upon award).
The contractor may not bring a HM onto MAFR, nor use a HM, until the contractor receives all https://usaf.learningbuilder.com/account/login/?ReturnUrl=%2f required authorizations. For definition of HM, the contractor shall refer to FED-STD-313D. HM includes pesticides and cleaning supplies. The contractor shall not use EPA-prohibited chemicals and shall be encouraged to not use products which result in the generation of hazardous waste, air or water pollution. Class I Ozone Depleting Substances shall not be used for any requirement of this contract. The contractor shall maintain a copy of current SDSs readily available to employees for HM in use or being stored. The contractor shall be OSHA compliant.
2.9.2. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws/regulations come into effect. The contractor shall notify the CO at earliest knowledge of the law/regulation change.
2.10. Notification of Environmental Spills. If the contractor spills or releases any substance contained in 40 CFR 302, Designation, Reportable Quantities, and Notification into the environment, the contractor or its agent shall immediately report the incident to Cannon AFB Fire Dept. at 911. The liability for the spill or release of such substances rests solely with the Contractor and its agent.
2.11. Safety Requirements. In performing work under this contract, the contractor shall conform to the safety requirements as required by OSHA. The contractor shall provide protection to Government property to prevent damage during the period of time the property is under the control or in possession of the contractor. The contractor shall record and report promptly (within one hour) to the CO or COR, all available facts relating to each instance of damage to Government property or injury to either contractor or Government personnel.
2.12. Contractor Manpower Reporting Requirements. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Air Force via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address:
http://www.ecmra.mil. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the help desk at:
