Performance Work Statement (PWS) For

Medical Delivery Courier Services

April 2021

1.0. Objective. The contractor shall provide all transportation, personnel, labor, supervision, and management to perform Medical Delivery Courier services for the 355th Medical Group (355 MDG), Davis-Monthan AFB (DMAFB), AZ.

1.1. Basic Services

1.1.1. Delivery from 355 MDG. The contractor shall provide specimen transport of frozen, refrigerated, or ambient samples and supplies from the 355 MDG to Government-designated local hospitals and referral laboratories in a timely manner. Government personnel will provide the contractor with the location of final destination(s) when the specimens and supplies are picked up by the contractor. The contractor shall pick-up specimens once a day at 3:00 p.m., Monday through Friday, from the Main Laboratory Shipping Department in building 400 on DMAFB, and make deliveries directly to the location(s) designated by Government personnel.

Government personnel will notify the contractor at least one day in advance, not later than 1:00 p.m., if services are not required on any specific day, and when the Main Laboratory is scheduled for closure or when there is a change in clinic operation and the pick-up time is moved earlier or later than 3:00 p.m.

1.1.2. Delivery to 355 MDG. The contractor shall also provide pickup of laboratory supplies and radiology items from various Tucson area locations and deliver items once a day to the Main Laboratory Shipping Department in building 400 on DMAFB at 3:00 p.m., Monday through Friday. Government personnel will provide the contractor with pickup locations via telephone before 10:00 a.m the day of pick-up.

1.1.3. On-Demand Services. The contractor shall provide on-demand deliveries as requested by designated Government personnel, during and outside of the 355 MDG working hours of 7:30 a.m. - 4:30 p.m. Upon contract award, the contractor shall provide the Contracting Officer with a point of contact and telephone number(s) that Government personnel will call to request on-demand requests. On-demand requests are billed separate and invoiced monthly in Wide Area Workflow (WAWF).

1.2. Delivery Requirements. The contractor shall perform deliveries in accordance with the Health Insurance Portability and Accountability Act (HIPAA), Occupational Safety and Health Act (OSHA), and Air Force Occupational Safety, and Health (AFOSH) standards.

2.0 Special Requirements

2.1. Continuation of Essential Contractor Services. The Government has determined that the services in PWS paragraphs 1.1. through 1.2., inclusive, have been determined to be mission essential, in accordance with the Defense Federal Acquisition Regulation Supplement (DFARS), SOLICITATION/CONTRACT NUMBER

Subpart 237.76, Continuation of Essential Contractor Services. The contractor providing essential services shall be prepared to continue providing such services, in accordance with the terms and conditions of their contract, during periods of crisis, as determined by Government personnel.

2.1.1. Essential Contractor/Liaison Services Plan. In accordance with DFARS 252.237-7024 provision, Notice of Continuation of Essential Contractor Services, offerors shall provide with their offer a written plan describing how it will continue to perform the essential contractor/liaison services listed in the PWS. The required contents of the mission essential plan can be found in the DFARS 252.237-7024 provision.

2.1.2. Contractor Mission Essential Employee Identification. Upon contract award, the contractor shall provide the name, address, and telephone number of a point of contact for purposes of continuation of essential DoD contractor services during crisis. The contractor shall also provide the names of contractor employees having military mobilization recall commitments, and have adequate plans for replacing those employees in the event of mobilization, in accordance with DoDD 1200.7., Screening the Ready Reserve, and DoDD 1352.1, Management of Regular Reserve Retired Military Members, which the contractor shall provide with their offer.

3.0. Services Summary

4.0. General Information

4.1. Listing of Employees. The contractor shall maintain a current listing of employees performing services on DMAFB. The list shall include employee's name, social security number, driver’s license number and state of issue, date of birth, and country of citizenship. The contractor shall submit the list to the Contracting Officer and the Contract Manager, prior to the contract start date, and submit an updated list when an employee's status or information changes, within 5 working days of the change. The name of any contractor employee that is terminated during the period of the contract must be reported to the Contracting Officer in writing within one workday of the employee’s termination.

4.2. Antiterrorism Awareness Training. Level I – Antiterrorism (AT) Awareness Training is available to all contractor employees. Non-Common Access Card (CAC) holders can accomplish Level 1 - AT Awareness Training at https://jkodirect.jten.mil/Atlas2/page/login/Login.jsf. Additionally, contractor employees may contact the Government sponsoring agency’s Unit Antiterrorism Representative (UATR) to request Antiterrorism Awareness Training material.

4.3. Reporting Requirements. Contractor personnel shall immediately report to an appropriate Government authority any information or circumstances of which they are aware may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor personnel are required to review suspicious activity reporting procedures. Contact the Government sponsoring agency’s Unit Antiterrorism Representative (UATR) to request Suspicious Activity Reporting training material.

Contact the Base Defense Operation Center (BDOC) to report actual suspicious activity at 520- 228-3200.

4.4. Incident Notification. Contractor personnel shall be alert for notifications from the base public address system (i.e. Giant Voice). Additional notifications can be received via the Davis Monthan AFB Facebook page. In the event of an incident, contractor personnel shall adhere to the guidance provided in the Antiterrorism Awareness Training.

4.5. Pass and Identification Items. The contractor shall ensure that all necessary pass and identification items required for contract performance are obtained for contractor employees and non-Government owned vehicles. Contractor Pass/ID’s are worked through the 355 MDG Contract Manager.

4.6. Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes from contractor employees that depart for any reason before the contract expires (e.g. terminated for cause, retirement, etc.) and turn them in to the issuing office. Once contractor employees are no longer employed, the contractor office shall notify the issuing office of the termination immediately.

4.7. Traffic Laws. Contractor employees shall comply with all base traffic regulations.

Contractor employees are subject to random vehicle speed control checks. Contractor personnel cited for speeding on the installation may suffer loss of base driving privileges, debarment from the base, or other administrative action. The use of seat belts is mandatory for all vehicle occupants. The use of cell phones is prohibited while driving on base, unless the phone is a hands-free cell phone.

4.8. Random Personnel and Vehicle Searches. Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base, and may result in loss of base driving privileges, debarment from the base, or other administrative action.

4.9. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor owned

PWS -19 Apr 2021 Page | 4 vehicle or privately owned vehicle while on Davis-Monthan AFB.

4.10. For Official Use Only (FOUO). The contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

4.11. Physical Security. Contractor employees shall safeguard all Government property and controlled forms provided to the contractor.

4.12. Contractor Personnel Travel on the Installation. All contractor personnel will limit their travel on the installation only to specific areas required for performance of services, specified break and meal areas, or travel directly to and from these locations.

4.13. Privacy Act. The Contractor shall ensure that its employees fully understand and comply with Title 5 of US Code, Section 552.a and DoD 5400.7-R, DoD Freedom of Information Act Program. These procedures must be followed to identify and safeguard reports and data accordingly. The contractor will ensure that Contractor employees are briefed annually on properly identifying and handling Privacy Act data/information.

4.13.1. Protection of Personally Identifiable Information. Personally Identifiable Information (PII) refers to information that can be used to distinguish or trace an individual’s identity either alone or when combined with other information that is linked or linkable to a specific individual.

PII may range from common data elements such as names, addresses, dates of birth, and places of employment, to identity documents, Social Security numbers (SSN) or other government-issued identity, precise location information, medical history, and biometrics. The contractor shall protect all PII encountered in the performance of services in accordance with Department of Defense Directive 5400.11, Department of Defense Privacy Program, and DoD 5400.11-R, Department of Defense Privacy Program. If a PII breach results from the contractor's violation of the aforementioned policies, the contractor shall bear all notification costs, call-center support costs, and credit monitoring service costs for all individuals who's PII has been compromised.

4.14. Place of Performance. The contractor shall primarily perform services in the greater Tucson, Arizona region, including performance on Davis-Monthan AFB.

4.15. Federal Holidays. The Federal Government observes the following holidays. (Note: If the holiday falls on a Saturday then the holiday will be observed as recognized by the Federal Government). The contractor shall not normally be required to provide routine services on holidays, or the actual day set aside for observation.

4.16. Medical Group Closures. Certain days are designated as goal days, also known as “ACC Family Days” or “355th Wing (355 WG) Down Days” during the year. These days do not require service from the contractor. The specific dates will be announced in advance to allow adequate planning.

4.17. Contractor Employee Appearance. Contractor employees shall present a professional, conservative, and neat appearance. Contractor personnel shall report for duty in a professional manner, in appropriate attire befitting the work environment, and have complied with socially acceptable standards of personal hygiene. While performing services, contractor employees shall be neat and clean (free of visible dirt and stains), well-groomed and appropriately dressed.

The contractor employee's clothing shall fit correctly to provide a professional, modest appearance, in keeping with normally accepted commercial standards of dress for the work being performed.

4.18. Communication. Contractor employees shall maintain open, professional, and respectful communication with Government personnel (military and civilian) and other contractor employees. The Contract Manger will report valid complaints in writing to the CO for action.

Valid complaints will be considered a performance failure. The contractor shall resolve all substantiated complaints immediately upon notification of the complaint. Contractor employees are required to identify themselves as a contractor at all times while on the job.

5.0. Government Provided Items and Services

5.1. Emergency Services. The Government will provide security forces services (call (520) 228-4444 for emergencies only), and fire protection services (call (520) 228-3333). There is no emergency room on DMAFB, therefore contractor personnel shall call (520) 228-3333 for medical emergencies.
