About this file
This document is a Response Template for a Request for Response (RFR) issued by the Commonwealth of Massachusetts Operational Services Division for COVID-19 Pooled Testing Services. The RFR seeks qualified bidders to provide pooled testing services, including regular delivery of testing kits, processing of collected specimens at a CLIA-certified laboratory, follow-up testing, delivery of test results within 48 hours, and training and technical assistance support to eligible entities. Bidders must meet minimum service requirements as outlined in the RFR and may propose alternatives that provide equivalent or better performance.
The RFR requires bidders to submit information on their organization, financial condition, conflicts of interest, and ability to meet the minimum service requirements. Bidders may also propose alternatives to any component of the minimum requirements. Responses must be submitted through the COMMBUYS system by the stated deadline. The Commonwealth will evaluate the responses and select contractors to provide the requested pooled testing services.

Document Title: COVID-19 POOLED TESTING SERVICES ASK "RFR Number/Document Number" \* MERGEFORMAT Agency Document Number: RFR HSP42 COMMBUYS Bid#: BD-21-1080-OSD03-SRC02-58650 February 18, 2021
RESPONSE TEMPLATE
ASK "RFR Number/Document Number" \* MERGEFORMAT
INSTRUCTIONS:
Submission of Responses/Deadline for Submission
Bids will be submitted solely through COMMBUYS by the submission deadline date as outlined in the RFR Document, as it may be amended.
1. Responses must be submitted according to these instructions. Failure to submit a response in compliance with these instructions may result in a reduction in points, or all or a portion of the response being disqualified. Due to the number of expected responses, and the competitiveness of this procurement, a response may be disqualified solely for not being submitted in compliance with these instructions.
2. The written RFR response must be submitted using this “RFR Response Template” so that all responses appear uniform and consistent for selection purposes and to enable posting on COMMBUYS once selection is completed.
3. This WORD document must be used and may not be altered, reformatted or changed in any way or the response will be subject to rejection. This document must be saved in a WORD format and not in .pdf so that the document may be modified during negotiations if necessary. Bidders may not save this document as a .pdf format. A .pdf format will subject the response to rejection. Attachments allowable as .pdf submissions will be specifically noted, if any.
4. Each item must be addressed specifically by entering information in the available space or via an attachment where applicable. If an item does not apply, the response must indicate "N/A" or “Not applicable” or any other appropriate explanation. If an attachment is requested but the submission section does not apply to the Bidder, the Bidder does not need to submit any information for that section. For example, if the Bidder is not proposing Alternatives as outlined in Section 4-B, the Bidder does not need to submit an attachment for this section.
5. The questions presented are the best guess of what information is needed to evaluate bidders and are not exhaustive. Bidders should be as comprehensive in responding to each answer as possible and include all relevant information and considerations to assist in the review of a response and demonstrate the full capabilities of the bidder and why the Commonwealth should select the bidder as a Statewide Contractor. If questions do not elicit all the information a bidder believes is necessary to evaluate the bidder, the bidder should add this information where most applicable, or under the general “value-added services” section.
6. The Response Template submitted by the Bidder should follow the naming convention, “Attachment 1 – Response Template_[bidder].”
Each Respondent must complete the following application in accordance with all submission instructions and submit the completed application, along with any attachments, as part of its response to the RFR.
