About this file
This document is a Cost Response Template associated with a Request for Response (RFR) issued by the Commonwealth of Massachusetts Operational Services Division for COVID-19 Pooled Testing Services. The RFR seeks pricing for test processing services, transportation of specimens, and logistical/support services to support pooled COVID-19 testing for eligible entities. Bidders must provide price ceilings that will remain firm for the contract duration. Pricing is requested for initial startup, monthly administration, per pooled testing kit, per follow-up PCR or antigen test, and various hourly rates for site coordinators and specimen collection. The RFR was originally issued on February 18, 2021 and amended on February 25, 2021.
Pricing provided in the response must adhere to specific instructions, including saving the document in Word format, addressing all items, and providing breakdowns or explanations as needed. Bidders may submit an optional 2-page attachment to detail any additional costs. This RFR and associated contract opportunity is specific to the state of Massachusetts.

COST RESPONSE TEMPLATE
Please Note: This is a single document associated with a complete Bid (also referred to as Solicitation) that can be found on www.COMMBUYS.com. Bidders are responsible for reviewing and complying with all Bid requirements. Bidders may contact the OSD Help Desk at COMMBUYS@mass.gov or at 1-888-MA-STATE for technical assistance with COMMBUYS. The Help Desk is staffed from 8:00 AM to 5:00 PM Monday through Friday Eastern time, as applicable, except on federal and state holidays.
Bidders with disabilities or hardships that seek reasonable accommodation, which may include the receipt of RFR information in an alternative format, must communicate such requests in writing to the contact person. Requests for accommodation will be addressed on a case by case basis. A Bidder requesting accommodation must submit a written statement which describes the Bidder’s disability and the requested accommodation to the contact person for the RFR. The SST reserves the right to reject unreasonable requests.
INSTRUCTIONS:
Submission of Cost Responses/Deadline for Submission Bids will be submitted solely through COMMBUYS by the submission deadline date as outlined in the RFR Document, as it may be amended.
1. Responses must be submitted according to these instructions. Failure to submit a response in compliance with these instructions may result in a reduction in points, or all or a portion of the response being disqualified. Due to the number of expected responses, and the competitiveness of this procurement, a response may be disqualified solely for not being submitted in compliance with these instructions.
1. The written RFR cost response must be submitted using this “RFR Cost Response Template” so that all responses appear uniform and consistent for selection purposes and to enable posting on COMMBUYS once selection is completed.
1. This WORD document must be used and may not be altered, reformatted or changed in any way or the response will be subject to rejection. This document must be saved in a WORD format and not in .pdf so that the document may be modified during negotiations if necessary. Bidders may not save this document as a .pdf format. A .pdf format will subject the response to rejection. Attachments allowable as .pdf submissions will be specifically noted, if any.
1. Each item must be addressed specifically by entering information in the available space or via an attachment where applicable. If an item does not apply, the response must indicate "N/A" or “Not applicable” or any other appropriate explanation.
1. If a Bidder’s proposed cost structure includes a cost outside of the components included in this cost attachment, the Bidder should describe those costs in the optional boxes below the relevant section.
1. If a Bidder wishes to consolidate two cost components into one component, it must provide a detailed breakdown of each part of the consolidated component in the optional text box. For example, if the bidder wishes to include shipping costs from Section B in the per pooled testing kit rate in Section A, it must break-out the pooled testing kit costs and the shipping costs in the optional text box in Section A.
1. If a Bidder does not have costs for a component include here, or if those costs are built into another cost component per above, the rate box for that cost component should be labeled as “N/A”.
1. The Response Template submitted by the Bidder should follow the naming convention, “Attachment 3 – Cost Response Template_[bidder].docx”
1. If a Bidder requires additional space to detail any of its costs, it may submit an Attachment of no more than two pages to describe those costs. The Bidder should follow the naming convention, “Attachment 3 – Cost Response Template ATTACHMENT_[bidder].docx”
