The form implements Public Law 115-232 requirements through Federal Acquisition Regulation (FAR) provisions 52.204-24 and 52.204-26, and contract clause 52.204-25. Vendors must complete two representations: (1) whether they will provide covered telecommunications equipment or services to the Government in performance of any contract, subcontract, or other contractual instrument resulting from the solicitation, and (2) whether they use covered telecommunications equipment or services, or any equipment, system, or service incorporating such covered items. Vendors responding affirmatively to either representation must provide additional disclosure information as specified in the FAR provisions. The completed certification must be signed by a merchant's authorized representative with binding authority and returned to the contracting officer's Contract Specialist. This is a standard compliance document template with no specific products, services, pricing, or award information.
