PERFORMANCE WORK STATEMENT

REPAIR & PREVENTIVE MAINTENANCE

FOR GYM EQUIPEMENT 28 July 2023

I. Scope of work: contractor shall furnish all experience, labor, tools, equipment and transportation necessary to perform repair of all cardio, selectorized and free weigh equipment located at Chace fitness Center and Lodging, at Lajes Field, Azores.

II. Certifications: Contractor shall have at the minimum 2 years experience in fitness equipment maintenance or related repair in the same career filed. Must provide documentation to support certification. Contractor must speak and write in English.

III. Schedule of work: contractor work may not exceed 50 hours per month. Upon notification of repair work contractor has 3 calendar days to identify the problem and make the required repairs or replacement of defective parts.

Scheduled Hours: Maintenance should be performed between the hours of0800 – 1700 Monday – Friday, 0900 – 1700 Saturday/Sunday the exception of Christmas Day and New Year's Day.

IV. Specific Tasks: Contractor shall provide repair of all cardio, selctorized and weight training equipment located in the Fitness Center and Lodging. Contractor may have to perform up to 50 hours per month, either for repair or preventive maintenance. Services include but are not limited to:

1. The contractor shall repair all equipment to include repairs and replacement of defective parts and removal from floor of all non-working or unsafe equipment until proper repairs are made and equipment is operational and ready for customer use.

2. Fitness Center will maintain records of each machine to include all repairs completed and/or attempted. All entries should be dated and man-hours required recorded. This information will provide a history for each machine to be used in considerations for budgeting, reordering and replacement of machinery. Records will remain the property of, and are subject to, the exclusive control of the US Government. Log books will be completed and kept at the front counter of each facility.

3. Contractor will be provided work/log sheets to monitor hours and repairs against each requested repair. Contractor shall log in each time he works on that particular request.

Each sheet will be the time sheet of record for the Fitness Center.

4. The contractor is prohibited from any of the following practices:

a. Repairing machinery not belonging to the Fitness Center and Lodging.

b. Working additional time without approval from the contracting Officer.

c. Using unauthorized persons to repair or perform maintenance on equipment. All designated subcontractors, must have the appropriate credentials and documentation on file at the Contracting Office.

d. Is not allowed to store unauthorized chemicals in the facility.

e. Contractor shall adhere to local lock-up procedures for equipment repair room security when completing required work.

V. General Information:

1. Invoicing: The Fitness Center Director or designee are responsible for the inspection and acceptance of services authorized. All work shall be accomplished in a timely and professional manner. Work, which does not conform to the specifications contained herein, will be justification for rejection. Correction of any deficiency shall be at no cost to the United States Government.

2. Payment: Payment will be paid monthly upon receipt of properly certified invoice for services rendered under this contract. The Fitness Center Director or designee may observe contractor at any time to assure that the proper repair is being performed in accordance with established guidelines.

3. Leave of Absence: In the event the contractor anticipates a leave of absence, the Contractor shall provide a suitable employee/subcontractor to perform required maintenance in his/her absence at least 7 calendar days before absence. Appointment of this person must be coordinated with and agreed upon by the Contracting Officer. All required credentials and documentation must be on file at the Contracting Office and Fitness Center prior work performance by the employee/subcontractor. Planned absences will not exceed 14 days during the contract period.

4. Government Furnished Property: the government will grant use of the manufacturer’s owner’s manual for machinery and maintenance guidelines, at no cost to the contractor.

The Government will provide all parts for repair and maintenance.

5. The contracting officer may in writing required that the contractor be removed from the work site, area, or base due to objectionable behavior. Objectionable behavior includes wearing inappropriate attire, using profane language, or abusing or disobeying base/host country rules or regulations.
