About this file
This document is a Bidders Authorized Representative Form issued by the State of Oregon, Department of Forestry (ODF), for procurement S-62900-00014481 related to Cone Collection Climbing Services in the Klamath Lake District. The form must be completed and returned with bids, signed in ink by an authorized representative of the bidding organization. Bidders are required to provide their legal name for tax filing purposes, mailing address, business designation (corporation, partnership, LLC, sole proprietorship, etc.), federal tax ID number, and authorized representative contact information including phone, fax, and email. The solicitation establishes a proposal submission deadline of August 6, 2025, with the RFQ posted on July 29, 2025.
The form requires bidders to declare their residency status as either Oregon resident or non-resident bidders, with specific definitions tied to unemployment tax or income tax payments within Oregon during the twelve months preceding bid submission and maintenance of a business address in the state. Non-resident bidders must identify their state of residency and disclose any preference percentages or preferences they receive in their home state for government contracts, which Oregon law requires be added as a percentage increase to their bid for evaluation purposes. Bidders must certify that they have not discriminated against minority, women, or emerging small business enterprises or disabled veteran-owned businesses in subcontracting decisions, maintain written policies compliant with HB 3060 (2017) regarding prevention of sexual harassment and discrimination, and are not in violation of Oregon Tax Laws as defined in ORS 305.380(4). The form serves as an attestation that bidders have read and understood all solicitation provisions, prepared bids independently without collusion, possess adequate resources to perform required services, and agree to comply with all contract terms upon award.
