About this file
This document is a Past Performance Questionnaire (PPQ) that will be used to evaluate the past performance of an offeror being considered for a federal contract opportunity.
The PPQ requests information about the offeror's name, address, point of contact, and role on a past contract. It also asks the evaluator to provide details on the past contract, such as the contract title, number, type, location, award and acceptance dates, original and final contract prices, and a project description. The evaluator is then asked to rate the offeror's performance in several areas including quality, schedule/timeliness, communication, management/personnel/labor, cost/financial management, and subcontract management. The evaluator is also asked if they would hire or work with the firm again and to provide an overall rating.
The PPQ is related to Solicitation Number 36C77624R0140 for a Design Build project to Replace the Storage Building at the Iowa City VAMC, which is being procured by the Department of Veterans Affairs Technology Acquisition Center Austin.
