STATEMENT OF WORK (SOW)
REMOVE CARPET, INSTALL NEW CARPET, BUILDING 1406, MAXWELL AFB
Introduction:
The Government has a requirement for a contractor to replace carpet in building 1406. The contractor shall furnish all necessary equipment, labor, supervision, and materials specified in this contract. The work is subject to the terms and conditions of this contract and as specifically cited in the Contractor’s Proposal to the Government.
1.1 The Contractor shall furnish and provide for the Government the quality and quantity of service per Section 4 of this Statement of Work.

3.0 Applicable Specifications/Standards/Related Studies:
APPLICABLE PUBLICATIONS, LAWS, REGULATIONS, ETC: The contractor shall comply with all State and local ADEM codes. Additional codes, standards, and guidelines are provided in the specification sections that are part of this contract.
4.0 Task Description:
4.1 LOCATION: This contract is to be performed building 1406 on Maxwell AFB, AL.
4.2 DESCRIPTION OF SERVICES: Contractor shall furnish all necessary equipment, labor, supervision and material to accomplish the work in this contract.
4.3 DESCRIPTION OF WORK:
4.3.1 Contractor shall survey area where work is to be accomplished to ensure accurate measurements are taken.
4.3.2 If asbestos containing material is suspected the carpet installer will hire a certified tester to evaluate asbestos tile as required and develop/implement mitigation plan for controlling hazard and managing either removal or encapsulation IAW with appropriate federal regulations and EPA.
4.3.3 Contractor shall remove carpet, and prep floors (patch/repair as needed to accommodate adhesion). Recycle existing carpet.
4.3.4 Replace floor covering with new carpet.
4.3.5 Remove existing vinyl or rubber base.
4.3.6 Move furniture to install flooring.
4.3.7 Contractor will install transitions between flooring of different types.
4.3.8 Provide and install new 4” Rubber Base (color to be chosen by the using agency) to include touching up paint on wall where the Cove Base was removed.
4.3.9 Contractor shall maintain a clean, hazard free, safe workplace at all times and ensure the site is secure & free of safety hazards at the end of each day. All debris shall be removed & disposed of off base.
4.3.10 Contractor will be responsible for removing/replacing furniture as needed the using agency will be responsible for personal items be removed from the furniture.
4.3.11 Contractor shall coordinate “after hours” access requests to the facility through the Technical Monitor for coordination w/the requestor.
4.3.12 Contractor shall label all equipment to include port-o-lets, trailers, storage containers, etc., that may be “staged” on base during the duration of this project. All above mentioned items will have contractors name & phone number(s) visible & legible.
4.4 PERFORMANCE OF WORK: Services shall be conducted in conformity with applicable regulations to contribute maximum satisfaction to responsible base offices. Contractors must abide by the Government- Exhibits A - L for the Construction Contract. Services shall be made with minimum divergence from schedules once established. All services shall be made with minimum disturbance and maximum protection of property. The PM/STM shall be notified 72 hours prior to work starting.
4.4.1 The contractor must within 24 hours of notice that a delay will be incurred notify the buyer in writing. This letter must identify why there will be a delay, and how long.
Also, the contractor shall take any actions in his control to prevent delay from occurring.
4.5 SUPERVISION: Contractors shall provide competent supervision at all times when work is in progress. Contractor is responsible for scheduling and coordinating various trade activities. The Contractor is also responsible for assuring that all work is accomplished, and materials used are in accordance with industry standards
4.6 ACCESS: The Government shall have access at any, and all times to the contractor's equipment, materials, supplies, assigned areas and sites of operation for inspection purposes.
4.7 PERFORMANCE TIME: 30 days after the ‘Notice to Proceed’ has been signed.
4.8 HOURS OF OPERATION: The Contractor will be able to accomplish this work between the hours of 7:00 AM and 5:00 PM. The Contractor will not normally be permitted to work on weekends or on the following legal holidays (or the day the Federal Government observes these holidays) unless he has coordinated such work with the contract representative at least seventy-two (72) hours in advance:

If interruption of any building utility services such as but not limited to the following, chilled water, electrical power, domestic water, fire protection water supply, or communications are required, the subcontractor shall perform the work after 5:00 PM and before 7:00 AM on weekdays or normal work week hours during the weekend.
The contractor shall coordinate the interruption of the utilities 30 days in advance with the STM and User. The contractor shall include any additional to the cost of performing the work in their original bid.
4.9 GROUND FAULT CIRCUIT INTERRUPTERS: Whenever contractor uses portable electrical tools or equipment in an outside location where operator will be in contact with a grounded surface or in an interior wet location where floor is conductive such as concrete, contractor shall provide and use a portable ground fault circuit interrupter (GFCI). This shall apply wherever electric power is supplied through government-owned facilities. Contractor shall be responsible for maintaining the GFCI in operating condition and testing it before each use.
4.11 INSPECTION AND ACCEPTANCE: Inspection and acceptance of Services will be performed at Maxwell AFB, Alabama by A BOS Subcontract Technical Monitor (STM). Inspection and acceptance will be conducted in accordance with this SOW and the Maintenance Service and Repair Agreement provided by the Contractor in their proposal.
4.12 COMMUNICATIONS CABLING PROTECTION: Contractor shall protect all communications cabling while working and if damaged is caused by the contractor than the contractor shall replace the damaged communications cabling with same or for internal cable base standard CAT6 cable. All CAT6 cable must be home run to communications room. Copper tie cables must be replaced with same. Fiber optic cables must be a continuous strand with no splices. All communications cabling must be wired to the T568B standard.
5.0 Travel Requirements: Reserved
6.0 Data/Reporting Requirements:
6.1 INSTALLED EQUIPMENT: Contractor will furnish manufacturer warranties as needed for any component incorporated into this project such as equipment; any component installed in or made a part of the existing structure and any exterior or interior finishes, etc.
6.2 AS BUILT DRAWINGS: The contractor shall provide as-built drawings regardless of any deviation from the original design.
6.2.1 If the contractor provides the original design plans, they will submit those plans (whether changes made or not) at the end of the contract marked and signed as ‘As- Builts’. The drawings will be delivered in a useable electronic format as an AutoCAD “.dwg” file, 2007 or greater.
7.0 Deliverables:
7.1 A 1 year warranty on material and labor.
7.2 The manufacturer’s warranty, if applicable.
8.0 Government Furnished Property/Facilities/Equipment: Reserved
9.0 Subcontract Technical Monitor:
9.1 The Technical Monitor (TM) is a Maxwell BOS employee assigned to interface with the contractor. The TM is authorized to provide technical direction within the scope of the contract.
All technical direction shall be issued to the contractor via a Contract Technical Directive (STD) approved by the Contracting Officer (CO).
9.2 The TM can assist the contractor in the interpretation of technical requirements and performance of the effort required. The TM shall have no authority to impose additional requirements or to change or delete existing requirements of the contract. Any clarification provided by the TM concerning the work to be performed shall not be construed as a change to the contract. Any changes to the original Scope of Work must be approved by the CO in writing prior to work being performed.
9.3 INSTALLATION REQUIREMENTS: As prescribed by the DAFFAR 5352.242-9000, Contractor access to Air Force installations, AFI 31-218, Motor Vehicle Traffic Supervision, and DoDM5200.08v3_AFMAN31- 101v3, Installation Perimeter Access Control.
9.3.1 The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.
9.3.2 The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver’s license, current vehicle registration, and valid vehicle insurance certificate to obtain a vehicle pass. Government ID card holders may not use their credentials to sponsor an individual on to the base to perform work that has been contracted to an official contractor.
9.3.3 During performance of the service, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials for any employee who no longer requires access to the work site.
9.3.4 Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office.
9.3.5 Failure to comply with these requirements may result in withholding of final payment.
9.3.6 Vehicle Inspections. All commercial vehicles will be directed to Gate 3 (Kelly St) or Gate 4 (Congressman
Dickenson for Gunter) for processing through the Commercial Vehicle Inspection (CVI) Area, when operational. During non-duty hours and holidays, commercial vehicle inspections will be conducted at Gate 1 (Maxwell Blvd) or Gate 4 only. Inspection members will conduct thorough inspections of the interior and exterior of the vehicle for items prohibited from the installation such as explosive devices, weapons and ammunition, drugs, and open or closed alcohol containers.
10.0 TRAINING REQUIREMENTS: N/A
11.0 STANDARD WARRANTY: The contractor shall provide any Original Equipment Manufacturer (OEM) standard commercial warranty applicable to the products being purchased at no additional cost to the Government.
11.1 Prior to project acceptance, the contractor shall provide the customer with all manufacturer warranties as needed for any component incorporated into this project such as equipment manuals, literature, CDs and DVDs and other documentation on all equipment and software.
11.2 Prior to project acceptance, the contractor shall provide written procedures, points of contact within company, and required information for all warranty services prior to project acceptance. This information shall include written confirmation of the warranty period, phone numbers and contact information/procedures for technical support, troubleshooting assistance, and replacement of faulty equipment and software.
12.0 GENERAL INFORMATION: Items must be brand new. Refurbished items are not authorized. NOTE: Add additional paragraphs, as needed, to describe your requirement.
13.0 CONTRACTOR NOTIFICATION RESPONSIBILITIES: The contractor shall notify the contracting office within 30 days before on-base performance of the service. The notification shall include:
13.1 Name, address, and telephone number of contractor representatives.
13.2 The contract number and contracting agency.
13.3 The location(s) of service performance and future performance, if known.
13.4 The date service performance begins.
13.5 Any change to information previously provided under this paragraph.
14.0 REPORTING REQUIRMENTS: The contractor shall comply with AFI 71-101, Volume-1, Criminal Investigations, and Volume-2, Protective Service Matters, requirements. Contractor personnel shall report to 42d Air Base Wing Information Protection Office, any information, or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment and as required thereafter.
15.0 TRAFFIC LAWS: The contractor and their employees shall comply with base traffic regulations set forth in AFI 31-218, Motor Vehicle Traffic Supervision and DoDM5200.08v3_AFMAN31-101v3, Installation Perimeter Access Control. Personnel in violation may be issued a Central Violations Bureau Form 1805 traffic ticket.
16.0 RANDOM INSTALLATION ENTRY/EXIT CHECKS: Entry/exit vehicle checks are conducted by order of the 42 ABW Commander. These checks are conducted for the purpose of safeguarding the base and protecting government property by discovering and seizing stolen property, classified information, and contraband.
Refusal to submit to an installation (entry/exit) vehicle check may result in the loss of base driving privileges, revocation of base registration, or debarment action.
17.0 CELLULAR PHONE OPERATION POLICY: The use of cellular phones while operating a motorized vehicle is prohibited on Maxwell-Gunter. Although discouraged, drivers are authorized to use devices, i.e., ear bud or ear boom, which allows their cellular phone to be operated hands-free. The device must not cover both ears. This policy applies to everyone driving on Maxwell AFB and Gunter Annex. Personnel in violation may be issued a Central Violations Bureau Form 1805 traffic ticket.
18.0 WIRELESS ELECTRONIC DEVICES: The contractor will not establish their own Information Technology (IT) systems or networks (Local Area Networks [LAN], Wide Area Network [WAN], Cellular phone/USB Modem as WAN, Wi-Fi as WAN, etc.), or camera system without the direct permission of the Program Manager and governing communications and responsible information systems office (42 CS).
19.0 FIREARMS AND AMMUNITION: Transporting weapons or ammunition, concealed or otherwise, IS NOT permitted by any non-law enforcement personnel on Maxwell AFB/Gunter Annex at any time regardless of state issued concealed weapons/LEOSA permits. Violations may result in criminal prosecution under the applicable federal laws.
20.0 ILLEGAL WEAPONS: The below weapons are considered illegal, unless specifically authorized by competent authority, and are prohibited on Maxwell AFB/Gunter Annex. Violations may result in criminal prosecution under the applicable federal laws.
20.1 Switchblade knives or knives with any type of automatic blade release.
20.2 An incendiary/explosive weapon (e.g., grenades, flash bangs).
20.3 Fireworks
20.4 Homemade mortars, aka "tennis ball launchers" or similar devices.
