About this file
This is a Depot Repair Cost Format template (CDRL A009 Attachment 0012) for reporting maintenance and repair expenses associated with weapon system components under a performance-based logistics contract. The template is designed to capture comprehensive cost data for the Apache AH-64 Fire Control Radar (FCR) system under contract W58RGZ-26-R-0004, awarded by the Department of the Army Materiel Command Contracting Command at Redstone Arsenal. The spreadsheet requires contractors to report production repair costs, material and labor expenses, repair quantities, scrapping costs, and supporting overhead allocations across four cost buckets: Storage, Transportation, Engineering, and Other.
The template mandates specific data requirements including per-unit and total material costs, labor costs calculated as three-year averages, total production and repair quantities over a three-year period, and scrapping quantities and costs. All components provisioned with B17 Source of Supply codes and capable of unit requisition must be included, with costs reported upon completion and delivery per DD-250. Contractors must allocate Procurement and Repair Management costs appropriately across labor columns and include New Equipment Obsolescence Forecast (NEOF) labor costs in the three-year average calculations. Supporting documentation detailing all costs within the four specified buckets must accompany each submission, with the template effective as of 29 January 2026.
