Attachment 001 Performance Work Statement and QASP
Device Service Plan, Fort Pierce FL
GENERAL INFORMATION
1. Scope of Work:
1.1. Laboratory Device Service Plan for ICAP Pro XP Duo, Serial Number ICAPPRO60238.
1.2. Device is located at a USDA Facility, 2001 S Rock Road, Fort Pierce FL 34945
1.3. Period of Performance: Base period plus two option periods.
2. Definitions:
2.1. CLIN – Contract Line Item, numbered. Established in the solicitation, will start with
“0001.”
2.2. Business Days – Standard business days with holiday exceptions as noted:

CONTRACTOR PERFORMANCE REQUIREMENTS
3. Technical Requirements/Tasks
3.1. The contractor shall provide a ICAP Pro XP Duo device, Serial Number ICAPPRO60238, service plan for all CLIN periods that includes:
3.1.1. One on-site annual preventative maintenance visit per CLIN period including any preventative maintenance scheduled in advance with USDA technical point of contact.
3.1.2. Software and firmware updates occurring at the same time as preventative maintenance or repair visit(s)
3.1.3. Technical support priority status with response performance as stated below:
3.1.3.1. Technical support response to inquiries within 24 hours of initial contact.
3.1.3.2. Corrective repair onsite arrival response within three to five business days of initial contact.
3.1.4. All device repair expense coverage including parts, travel, labor, and shipping/handling.
3.1.5. Access to the OEM Manufacturer of the serial numbered device online laboratory device related knowledge base to assist operator with method development as well as answer any questions that pop up regarding sample up take, method issues, line selection, etc.
3.2. Service report for all service visits to be delivered to the USDA technical point of contact within five business days of any inspection visit.
4. Contractor’s Key Personnel: The contractor shall provide a repair service technician that is authorized by the OEM manufacturer of the serial numbered equipment detailed in Section 3.1 above to perform and document the service requirements.
5. Security Requirements: All contractor personnel shall always be in the presence of a USDA escort while on location.
6. Quality Assurance Surveillance Plan (QASP): The Government shall establish a QASP as a means of evaluating whether the Contractor is meeting the performance standards/quality levels identified in the PWS to ensure that the government pays only for the level of services received.
