About this file

This is an Installation Exceptional Family Member Program (EFMP) Document Checklist and supporting materials for EFMP respite care eligibility at Fort Bragg.

The checklist identifies six required documents that must be submitted for EFMP respite care services: a Garrison Commander's Letter (dated and signed), a Respite Care Panel Report (dated with family's civilian email address if available), DA Form 5189 (Application for Respite Care for Children and Adults with Disabilities with all fields completed), DA Form 4700 (Medical Record-Supplemental Medical Data signed and stamped by Primary Care Manager), a Family Needs Assessment, and an Authorization for Transferring EFMP Respite Care Files to the government contractor. The package includes example documents demonstrating approval of 25 hours monthly respite care at level four provider services, valid through December 20, 2023. DA Form 5189 requires comprehensive information about the individual with disability including identifying information, medical history, medication, allergies, seizure history, functional capabilities, and special care instructions. DA Form 4700 documents medical eligibility based on eight criteria including limited self-help skills, severe seizures, neurological impairment requiring assistance with activities of daily living, tube feeding, tracheotomy with frequent suctioning, apnea monitoring, behavioral safety issues requiring constant supervision, and life-threatening chronic conditions requiring frequent hospitalization. The Family Needs Assessment gathers information about family resources, current support systems, activities, concerns regarding care, behavioral considerations, and other family members in the program. The Authorization form allows release of respite care files to the government contractor (Strategic Resources, Inc.) providing respite care services.
