About this file
This is an excavation permit application and approval packet for the U.S. Army Garrison Hawaii that details the requirements and procedures for any excavation work within USAG-HI cantonment areas, road and utility rights-of-way, training facilities, and range areas.
The permit applies to military personnel, civilians, tenants, contractors, and utility operators performing any soil excavation, no matter how small or shallow. Key requirements include: obtaining approval from DPW Engineering Division, submitting completed forms to the Contracting Officer and DPW Engineering, coordinating with multiple utility agencies (including Aqua Engineers, Gas Company, Hawaiian Electric, AT&T, Hawaiian Telcom, Spectrum Cable, and others), conducting utility toning when required, maintaining the permit on-site during excavation, and obtaining environmental clearances through a Record of Environmental Consideration (REC). The permit is valid for 90 days after the period of performance end work date, and utility toning approvals are generally valid for 90 days unless otherwise coordinated. The document emphasizes that failure to locate underground utilities and environmentally sensitive areas can result in dangerous situations, work interruptions, and potential regulatory fines.

EXCAVATION/DIGGING PERMIT
Applies to Military, civilians, tenants, contractors, utility operators or any other entity per-forming any excavations of soil (no matter how small or shallow) within USAG-HI canton-ment area and/or any road and utility right-of ways, training facilities/complexes and range area.
Failure to locate underground utilities (water/sewer, electric, gas, phone, steam/chill water, control cables, petroleum/POL, cable TV) and environmentally sensitive areas (endangered species, archeological sites, contaminated sites) can cause dangerous situations, work inter-ruptions and could lead to potential fines and penalties from regulatory agencies. Excava-tion may also generate storm water discharge issues (contact DPW Environmental Storm Water Program for clarification).
Step 1: Obtain an excavation permit from DPW Engineering Division and complete all re-quired coordination stipulated in the permit prior to begin work.
Step 2: Submit a completed form to the Contracting Officer assigned to the project and to DPW Engineering Division prior to any excavation.
Step 3: The application becomes a certified excava-tion permit after all utility agencies have approved the appropriate sections and the Contracting Officer or Project Manager has signed the permit.
Step 4: Maintain the permit on site at all times dur-ing excavation.

EXCAVATION PERMIT
Request and Approval Packet
An approved excavation permit is valid 90 days after period of performance end work date. If upon the expiration date and work has not begun, the contractor, unit or individual is required to submit a new permit.
1. Instructions:
a. A Facilities Engineering Work Request (FEWR) is mandatory for any work on Army installations. Ensure a signed DA4283 is submitted to DPW prior to any work.
b. A Record of Environmental Consideration (REC) is a separate process, with its own form, conducted by DPW Environmental (see item 12, page 5). Depending on circumstances on the ground, this process can take some time. Therefore, we highly recommend you complete this process BEFORE starting the EP process. Not doing as recommended could jeopardize your POP date.
c. Site maps and/or detailed drawings are required for processing and coordination. This permit approves excavation only in the area(s) depicted on your drawings.
d. Should you elect to not conduct toning where indicated, you become liable for all costs, fees and repairs necessary to restore interrupted facilities, service outages or damages to Army facilities.
e. Extremely large or consolidated permits are not encouraged as processing can take longer time to complete and impact other excavation permits from timely completion.
f. Early coordination with 30th Sig Bn/NEC is highly encouraged to improve and shorten processing time.
g. The Army does not currently subscribe to the One-Call service.
i. Joint Trunking System (JTS) is an Army owned and controlled telecommunications system that resides on and off all DOD bases. JTS is not supported under the One Call System.
ii. Should Contracting Officer (KO) require the use of One Call, please contact the approving authority for more specific instructions.
h. Any permit with 40+ pages of drawings shall have the drawings submitted on CD.
i. TONING APPLICATIONS WITH SERVICE PROVIDERS ARE ONLY GOOD FOR 90
DAYS UNLESS COORDINATED OTHERWISE.
j. Requestor must provide entire Scope of Work (SOW) including on and off post work connected with the proposed project.
