About this file
This is a First Article Test (FAT) Waiver Worksheet template used by contractors to request exemptions from first article testing requirements on federal contracts. Contractors must submit the form on company letterhead and signed by a company agent, with the Contracting Officer available for questions regarding submission requirements.
The worksheet requires contractors to provide current acquisition information including the contracting officer's name, solicitation/contract number, contractor name and CAGE code, item nomenclature, part number, NSN, top-level drawing number, and revision level. Contractors must specify whether they are requesting a partial waiver (specific tests only) or full waiver (all testing), with precise TDP references for any tests to be waived. Justification options include: (A) successful completion of a FAT on a recent contract for the same item using identical facilities, equipment, processes, and subcontractors; (B) successful FAT on a similar item tested to equivalent requirements; (C) continuous material production with supporting documentation; (D) successful FAT on a next-higher assembly to eliminate redundant lower-assembly testing; or (E) other justification with specific supporting details. The form also requires disclosure of any quality issues, manufacturing data changes, subcontractor changes, or Material Review Board actions related to the item. Section 6 requires the submitter's printed name, title, company name, address, phone number, and signature certifying the accuracy of all statements provided.
