About this file

This is a Qualifying Project (QP) Form (Attachment 0003) used to document and evaluate past performance on projects relevant to federal contract proposals. The form serves as a standardized template for offerors to submit evidence of completed work that demonstrates their technical capabilities and organizational capacity for proposed contract domains.

The form requires offerors to provide comprehensive project documentation including the specific contract/agreement number, contracting party point of contact information, period of performance, proposed domain alignment, and NAICS code classification. For Large, Emerging Large, or Small Businesses, qualifying projects must have at least one year of performance but not exceed four years from the final solicitation date; Commercial-Sector Vendors must submit complete projects. Offerors must identify which technical capabilities from Section 3.0 of the Performance Work Statement align with the submitted project and provide a detailed mapping on Page 2 demonstrating relevance to the proposed domain. The form requires certification of key performance metrics including vacancy rate, average time to fill rate for positions, passthrough rate for subcontracted labor, schedule rate compliance (for Commercial-Sector Vendors), and documented efficiencies. Additional requirements include dollar value documentation, CAGE code information if different from the offeror's primary code, legal entity attestation, and certification by both the contracting official and the offeror. The form distinguishes between Labor-of-Effort (LOE) and Outcome-Based qualifying projects, and the Government reserves the right to contact the provided point of contact to verify all submitted information.
