About this file

This is a Past Performance Assessment Questionnaire template used to evaluate contractor performance on previous contracts. The questionnaire consists of three parts: Part I requires contractors to provide contract identification details including contract number, value, and dates; Part II contains the actual performance evaluation with ratings across five areas (compliance, project management, timeliness, cost control, and customer satisfaction) using a four-tier rating system from "Substantial Confidence" to "No Confidence"; and Part III collects respondent information.

The questionnaire is related to a pre-solicitation notice for CBRND Professional Services support for JPM Sensors (solicitation W911QY25RJS01) to be released December 2024. This will be a small business set-aside task order under the OASIS+ Small Business Management & Advisory Domain (NAICS 541611) contract vehicle. The task order will use a hybrid contract structure with primarily Firm-Fixed-Price CLINs and some Cost Reimbursable CLINs for travel and ODCs. Questions are due by December 20, 2024 at 5:30 PM ET to the specified contracting officers. The contracting agency is the Army Contracting Command at Aberdeen Proving Ground.
