PAFB Drinking Water Reporting and Record Management
SOP
Monthly Operating Reports (MORS)
· These reports are hand delivered to 45th SW Civil Engineering Squadron/Environmental Quality Office (45 CES/CEIE) by from Ron Burgdorf with the 45th Civil Engineer Squadron/Civil Engineering Operations Infrastructure (CES/CEOI) by the 5th of each month. Review the MOR for accuracy and submit to Florida Department of Environmental Protection (FDEP) website DEP_CD@dep.state.fl.us by the 10th of each month.
· Check accuracy for: Correct date and signature, PWS ID Number, Total, Average and Maximum finished water, Chlorine Residual Concentration and emergency or abnormal conditions and overall documentation.
· Scan the MOR document and save as adobe format and place in the share drive Y:\CEIE\Water Program\Water Program (PAFB)\Drinking Water\Monthly Operating Report\2019

Monthly Microbial Sampling (Micros)
· These reports are received from 45th Aerospace Medicine Squadron/Bioenvironmental Engineering “BIO” (AMDS/SGPB) by the 5th of each month. Review the Micros for accuracy and submit by email to DEP_CD@dep.state.fl.us by the 10th of each month.
· Check accuracy for: Documentation legibility, Lab Receipt and Analysis Date, PWS ID Number, Sample Points, Combined Chlorine Residual cannot be < 0.6 mg/L, pH, Analysis Method, Total Coliform Column, Lab Sample Numbers, Correct Lab Signature/Date and overall documentation.
· The Total Coliform Column for each sample point should show an “A” for absence of Total Coliform. However, if a “P” for Positive is shown in the column, BIO will re-sample and forward the results to 45 CES/CEIE.
· Remove transmittal cover letter and save the Micros document as adobe format and place in the share drive Y:\CEIE\Water Program\Water Program (PAFB)\Drinking Water\Micro\2019

Quarterly Total Trihalomethanes and Haloacetic Acids Five (TTHM & HAA5)
· These reports are received from BIO. The compliance laboratory results will be submitted to 45 CES/CEIE by the 5th of the 2nd month of each quarter. Review TTHM & HAA5 sampling results (exceedance if > 80 mg/L for TTHM or > 60 mg/L for HAA5) for accuracy and submit to DEP_CD@dep.state.fl.us Save as adobe format and place in the share drive Y:\CEIE\Water Program\Water Program (PAFB)\Drinking Water\TTHM Quarterly Compliance\2019
· Go to sent file in email and save the email you sent in the same file.
· The quarterly reports (FDEP forms) supporting the laboratory results will be completed by BIO and submitted to 45 CES/CEIE. Review TTHM & HAA5 results for accuracy and submit to DEP_CD@dep.state.fl.us Again, Save as adobe format and place in the share drive Y:\CEIE\Water Program\Water Program (PAFB)\Drinking Water\TTHM Quarterly Compliance\2019
· Go to sent file in email and save the email you sent in the same file.
· If an exceedance occurs, BIO will submit the Operational Evaluation Level (OEL) Report to 45 CES/CEIE. 45 CES/CEIE will submit the OEL Report to FDEP.

Quarterly Disinfectant Residual (Chlorine)
· The quarterly reports will be completed by 45 CES/CEIE and submitted to FDEP by the 10th of the month after the sampling quarter. Chlorine residuals should not > 4.0 mg/L.
· Review Disinfectant Residual Reports results for accuracy and submit to DEP_CD@dep.state.fl.us Save as adobe format and place in the share drive Y:\CEIE\Water Program\Water Program (PAFB)\Drinking Water\Disinfection Report\2019

Outages (Malfunctions and Planned) All malfunction and planned outage information includes the following:
1. Incident Report Form – 1st submittal
2. Do Not Drink Notice” (DNDN) – 1st submittal
3. Microbial Sampling Results – 2nd submittal
4. Rescission Notice – 2nd submittal
This information will be submitted to Manny Cardona, manuel.cardona@dep.state.fl.us with FDEP. Go to sent file in email and save the emails you sent. Malfunctions (emergency) - Notifications to FDEP are required within 24 hours, by phone and email, or noon the next business day (if a malfunction occurs on a weekend). 45 CES/CEIE will complete the Incident Report Form. BIO will prepare the “Do Not Drink Notice” (DNDN) and post notices. 45 CES/CEIE will submit both the Incident Report Form and DWDN to FDEP. The sampling results provided by Kennedy Environmental and Medical Contract (KEMCON) will be submitted to BIO. BIO will then submit the sampling results and Rescission Notice to 45 CES/CEIE. 45 CES/CEIE will send the sampling results and rescission notice to FDEP.
· Planned Outages – Same as malfunctions except the incident report information may come from a different source such as 45th Civil Engineering Squadron/Civil Engineer Flight (45 CES/CEN) or Contractor/Inspector.

· These reports are submitted from Cape Launch Operations and Infrastructure Support (CLOIS) to 45 CES/CEIE by the 5th of each month. Review the MOR for accuracy and submit to DEP_CD@dep.state.fl.us by the 10th of each month.

The sampling results will be submitted by CLOIS to 45 CES/CEIE. 45 CES/CEIE will then submit the sampling results and Rescission Notice to FDEP.
· Save the adobe document and place in the share drive \\52sxht-fs-pat5v\CEI\CEIE\Water Program\Water Program (CCAFS)\Drinking Water\Malfunctions & Planned Outages\2019.
· Planned Outages – Notification to FDEP must be submitted at least one business day prior to the outage by phone or email. Follow the same procedure for planned outages as malfunctions.
