PAFB Drinking Water Malfunctions/Planned Outages Procedures Malfunctions- Notifications to Florida Department of Environmental Protection (FDEP) is required within 24 hours or before noon the next business day after a weekend. However, verbal notification to FDEP should be made as soon as possible.

· In most cases 45th Civil Engineer Squadron/Civil Engineering Operations Infrastructure (CES/CEOI) will notify 45th SW Civil Engineering Squadron/Environmental Quality Office (45 CES/CEIE) and 45th Aerospace Medicine Squadron/Bioenvironmental Engineering “BIO” (AMDS/SGPB) that a malfunction has occurred. To complete the Incident Report Form for malfunctions the following information must be collected:

1. Date and time the malfunction occurred.

2. The location (include building number and/or street address).

3. Facilities and personnel affected by the malfunction.

4. Description of the malfunction (include but not be limited to size of pipe, and if applicable, water mains affected due to depressurization if < 20 PSI, or tapping water lines and/or any other pertinent information related to the malfunction).

5. Estimated time the repair will be completed and does the malfunction require a “Do not Drink Notice” (DNDN), bacteriological sampling.

6. Explain what corrective action is being performed.

· BIO will send a copy of the DNDN for 45 CES/CEIE approval. Once approved, create a new file in the malfunction and planned outage folder and save the documentation as a pdf, and inform BIO they can post the DNDN.

· When completed, 45 CES/CEIE will submit the Incident Report along with the DNDN via email to FDEP Manny Cardona, manuel.cardona@dep.state.fl.us and Brevard County Department of Health (BCDOH), Cynthia Lecky, cynthia.leckey@flhealth.gov per the regulatory requirement. 45 CES/CEIE will create a new folder with the date of the malfunction and store the report and the DNDN as PDFs on the 45 CES/CEIE share drive. Save all submitted emails regarding the report in the 45 CES/CEIE share drive folder you created as proof of notification.

· A minimum of two days of bacteriological sampling is required. If the sampling results are satisfactory, Kennedy Space Center and Medical Contract (KEMCON) will send the results as a pdf to BIO and 45 CES/CEIE. Save the results in the created folder.

· BIO will create the rescission notice, send it to 45 CES/CEIE for approval, and then post it at the appropriate facilities.

· Submit via email, the rescission notice and lab results to FDEP and BCDOH. Save the email as a pdf in the 45 CES/CEIE share drive created folder as proof of notification.

· All documentation is stored in folder: CEIE\Water Program\Water Program (PAFB)\Drinking Water\Malfunctions & Planned Outages\2019

· File naming convention example (03092019 Malfunction Building 802).

· Copy the following individuals on all emails related to malfunctions: Laurie Fisher, Karla Guerrero Oliveras, Major Thomas Witter, Lt. Casey Meyering, SSgT Markus Beil, AC1 Brian Cullen, Jim Kelley, Ron Burgdorf, Tom Price and Sarah Hausman.

Planned Outages- Notifications must be submitted at least one business day prior to the outage.

· Scheduled Utility Outages Form is generated and submitted for a project by 45 Civil Engineering Squadron (45 CES/CEN) to air force base organizations. POCs from these organizations review and if approved, sign and forward the form back to

45 CES/CEN.

· 45 CES/CEIE and BIO receive the approved planned outage form from

· 45 CES/CEN. 45 CES/CEIE will generate an Incident Report Form detailing the description of the outage, while BIO generates from the same information, if warranted, a “Do Not Drink Notice” (DNDN) and post where needed.

· To complete the Incident Report Form for planned outages the following information must be collected:

1. Date and time the outage occurred.

2. The location (include building number and/or street address).

3. Which facilities and personnel that are affected by the outage.

4. Description of the outage (include but not be limited to size of pipe, and if applicable, water mains are affected due to depressurization if < 20 PSI or taping water lines and any other pertinent information related to the outage).

5. Estimated time the repair will be completed and does the outage require a “Do not Drink Notice” (DNDN), bacteriological sampling.

6. Explain what corrective action is being performed.

· BIO emails the DNDN to 45 CES/CEIE for review. 45 CES/CEIE will review and submit the Incident Report along with the DNDN in an email to FDEP (Manny Cardona, manuel.cardona@dep.state.fl.us) and Brevard County Department of Health (Cynthia Lecky, cynthia.leckey@flhealth.gov) per the regulatory requirement. 45 CES/CEIE will create a new folder with the date of the outage and store the report and the DNDN as pdfs on the 45 CES/CEIE share drive. Save all submitted emails regarding the report in the new folder as proof of notification.

· After the project is completed, if bacteriological sampling is required, BIO will perform the sampling and KEMCON will pick up and deliver the samples to the contract lab for analysis.

· KEMCON will advise BIO if two consecutive days the bacteriological samples passed and send the results to 45 CES/CEIE.

· BIO will email the rescission notice to 45 CES/CEIE. 45 CES/CEIE will email both the lab results and the Rescission Notice to the regulatory agencies. Again, save all emails regarding the report and supporting documentation in the 45 CES/CEIE share drive folder as proof of notification.

· All documentation is stored in folder: CEIE\Water Program\Water Program (PAFB)\Drinking Water\Malfunctions & Planned Outages\2019

· File naming convention example (03092019 Planned Outage Building 802).

· Copy the following individuals on all emails related to planned outages: Laurie Fisher, Karla Guerrero Oliveras, Major Thomas Witter, Lt. Casey Meyering, SSgt Markus Beil, AC1 Brian Cullen, Jim Kelley, Ron Burgdorf, Tom Price and Sarah Hausman.
