About this file
This is an Offeror Checklist for the Federal Occupational Health (FOH) Medications, Vaccines, and Medical Supplies (MVMS) procurement solicitation 7571MN26R00052. The checklist establishes mandatory evaluation criteria organized in two phases, with Phase One consisting of gate requirements that determine proposal compliance and eligibility for further consideration.
Phase One mandatory gate requirements include: (1) Regulatory Compliance & Licensure—valid State Board of Pharmacy Wholesale Distributor licenses, DSCSA-compliant transaction tracking systems, FDA Establishment Registration, and cGMP compliance documentation; (2) Financial Capability—audited financial statements for two fiscal years demonstrating a Current Ratio between 0.8 and 1.2 with detailed revenue justification if needed; (3) Just-In-Time (JIT) & Warehouse-less Logistics—certified statement confirming ability to ship individual units with zero Minimum Order Quantities and no minimum dollar values; (4) Clinical Protocol Alignment—completed Formulary Capability Matrix showing 100% availability of Unit-Dose or Ready-to-Administer formats for all items marked "Critical"; (5) Cold-Chain Integrity—validated Standard Operating Procedures and proof of 24/7 temperature data logging during transit; and (6) Web-Portal Functionality—video demonstration or technical manual proving system can enforce Formulary-Only ordering and support up to 250 unique sub-accounts. The checklist further requires comprehensive documentation across twelve categories including company registration and SAM.gov compliance, corporate information package, pharmaceutical and vaccine regulatory compliance, quality management documentation, technical capability narrative, IT and cybersecurity package, past performance with minimum three references from the past five years, distribution and logistics documentation, pricing documentation including base and all options, financial responsibility documentation, required policies and programs, and proposal organization into four separate volumes (Technical, Business/Pricing, Past Performance, and Web Portal Demo video not exceeding 35 MB). Proposals are due September 25, 2026 at 12:00 PM Eastern Time, with questions due by September 16, 2026 at 5:00 PM Eastern Time.
