Bidder Response Form Transition & Re-Entry Services using the Healing to Hope Credible Messengers Mentoring Model

RFP #26-DCYF-OJJ-102

Agency/Organization:

Except for limits that are noted on the Bidder Response Form, Bidders may utilize as much space as is reasonably required to respond to each question, provided all questions are repeated and remain numbered and ordered. If additional pages are needed, they should be attached to the page containing the initial portion of the response to a question and should be marked clearly to indicate that they provide a continuation of Bidder’s answer to a specific numbered question. Bidders should not submit product brochures, white papers, customer testimonials, cut sheets, or other pre-prepared materials in response to any of the questions. Bidders must submit complete, well-organized explanatory answers that address all of the specific questions asked in the Bidder Response Form. Bidders should not assume that Evaluators will be familiar with their businesses before conducting the evaluation.

BUDGET INFORMATION

Instructions When completing your proposed budget narrative please be sure to identify how the budget expenditures will assist the program in achieving its desired outcomes. Please reference the explanations below to ensure your budget line items are allowable within this grant award.

Personnel These funds are for the salary and benefits of program staff directly associated with the project to be supported through this funding application.

Supplies The key word in determining whether an item belongs in the Supplies category is "consumable." If the item can be used up, then it is a supply item.

Other Services and Charges This category is for services other than Personnel, which are required in the administration of the project. Such services may include communication, advertising, and rentals. Expenses for staff training, such as workshop fees, may be included.

Equipment This category is for equipment costs that directly support the staff and/or the program, such as laptops, cell phones, software, and furniture.

Travel All travel costs are included in this category, including personal car mileage, airfares, per diem, hotel etc., and must adhere to all state and federal rates.

Contractual Any sub-contracts funded through this award. The important distinction to remember is that when an agency contracts with an individual (no matter what service is to be delivered) the cost is reported in Contractual, not in Personnel.

Indirect This category is for overhead costs such as utilities or administrative services such as contract or fiscal management, etc. The proposed funds in this category must be less than 15% of the total requested budget.
