About this file

This is a Resident Bidder Affidavit required by the Commonwealth of Kentucky for Solicitation/Contract #RFB 758 1900000034. The affidavit establishes the legal requirements and verification process for bidders claiming resident bidder status under Kentucky Revised Statutes 45A.494(2). Bidders must be authorized to conduct business in Kentucky and demonstrate continuous business operations for one year prior to and through the contract advertisement date. The affidavit must be executed under penalty of perjury and notarized before submission.

To qualify as a resident bidder, an entity must have filed Kentucky income taxes, made payments to the Kentucky unemployment insurance fund, and maintained an active Kentucky workers' compensation policy for the required one-year period. The bidding agency retains the right to request documentation substantiating any resident bidder claim, and failure to provide requested documentation upon demand will result in bidder disqualification or contract termination. The affidavit requires the bidder's signature, printed name, title, date, company name, address, and notarization with seal and expiration date of the notary's commission.
