About this file

This is a Resident Bidder Affidavit required for solicitation RFB 758 2000000225, applicable to bidders, offerors, and contractors seeking resident bidder status in the Commonwealth of Kentucky. The affidavit establishes the eligibility criteria that entities must satisfy to claim resident bidder status when submitting bids or proposals for state and local contracts.

To qualify for resident bidder status, entities must demonstrate that on the date the contract is first advertised or announced for bidding, they are authorized to transact business in the Commonwealth and have maintained continuous Kentucky business operations for one year prior to and through the advertisement date. Specifically, qualifying entities must have filed Kentucky income taxes, made payments to the Kentucky unemployment insurance fund, and maintained an active Kentucky workers' compensation policy during this one-year period. The bidding agency reserves the right to request documentation supporting a bidder's claim of resident status, and failure to provide such documentation upon request will result in bidder disqualification or contract termination. The affidavit must be executed under penalty of perjury and notarized before a notary public.
