About this file
This is a Required Affidavit for Resident Bidder Status document issued by an unnamed bidding agency for Solicitation/Contract #RFB 758 2100000171. The affidavit establishes the mandatory requirements for bidders, offerors, and contractors seeking to claim resident bidder status under Kentucky Revised Statutes Section 45A.494(2). Bidders must affirm under penalty of perjury that their entity is authorized to transact business in the Commonwealth of Kentucky and has maintained continuous compliance with state requirements for one year prior to and through the date of contract advertisement. These requirements include filing Kentucky income taxes, making payments to the Kentucky unemployment insurance fund, and maintaining an active Kentucky workers' compensation policy throughout the specified period.
The bidding agency retains the right to request supporting documentation from any bidder claiming resident bidder status, and failure to provide such documentation upon request will result in immediate bidder disqualification or contract termination. The affidavit must be executed under oath before a notary public, with the affiant providing their signature, printed name, title, date, company name, and complete address. This document serves as a critical compliance requirement for any entity seeking to qualify for resident bidder preferences and must be submitted as part of the bid or proposal package for this solicitation.
