About this file

This is a Resident Bidder Affidavit for Solicitation/Contract RFB 758 2000000357, applicable to state and local contracting opportunities in Kentucky. The affidavit is a required document for bidders, offerors, and contractors claiming resident bidder status under KRS 45A.494(2). Bidders must certify under penalty of perjury that their entity is authorized to transact business in the Commonwealth and has maintained continuous business presence for one year prior to contract advertisement, including filing Kentucky income taxes, making payments to the Kentucky unemployment insurance fund, and maintaining an active Kentucky workers' compensation policy on the date the contract is first advertised.

The bidding agency retains the right to request documentation supporting any claim of resident bidder status, and failure to provide such documentation upon request results in bidder disqualification or contract termination. The affidavit requires execution before a notary public with signature, printed name, title, date, and company information from the authorized representative of the bidding entity. This document serves as a verification mechanism to ensure that bidders meet Kentucky's resident bidder eligibility requirements prior to contract award.
