About this file
Summary
This is a training documentation form (DSS Form 2433) issued by the South Carolina Department of Social Services for the Adult Protective Services program, applicable to in-home care providers including Certified Nursing Assistants, Personal Care Assistants, and Homemakers. The form serves as a mandatory training log to track employee completion of required competency-based training prior to providing independent services. Employees must receive training in eight core areas: basic first aid, medication assistance, care services specific to client conditions (such as Alzheimer's disease or cognitive disabilities), confidentiality, documentation and recordkeeping procedures, ethics and interpersonal relationships, proper lifting and transfer techniques, and infection control techniques. The form requires documentation of the trainer's name or identification as online/electronic training, the date of completion, and the number of hours for each training topic.
In accordance with South Carolina Department of Public Health regulations, all Personal Care Assistants, Certified Nursing Assistants, and Homemakers must complete a minimum of five hours of annual training on one or more of the required training topics listed above. The completed form must be maintained in the employee's personnel file on-site and made available for inspection during South Carolina Department of Social Services monitoring visits. The form requires entry of employee name, hire date, provider business name, and employee position, with a structured table format for tracking each training requirement systematically.
