About this file
This is a Past Performance Questionnaire template for the IRV Exterior Repair contract (OC-2026-133499 / RFP 75F40126R00047). The questionnaire is designed to collect references and performance ratings from previous clients regarding a contractor's work on similar projects.
The form requests offeror and reference contact information, along with specific contract details including contract number, total contract value, period of performance, and whether the offeror served as prime or subcontractor. References must rate contractor performance across seven assessment elements using a five-level rating scale: Very Good (VG), Satisfactory (S), Marginal (M), Unacceptable (U), and Not Applicable (NA). The assessment elements include Cost/Price Control, Schedule Adherence, Performance, Quality, Business Relations, and Personnel Qualifications. For each element, references must select a rating and provide supporting narrative explanations. The questionnaire also requires an overall performance rating, identification of contractor strengths and weaknesses, and a recommendation regarding future award consideration. A certification statement at the end requires the reference to attest that all ratings and comments are accurate to the best of their knowledge.
