About this file

The document is a Contractor Work Surveillance Checklist for the placement or removal of empty rented Rolloff Box (ROB) or Vacuum Box. The checklist provides a detailed inspection framework for government representatives to evaluate contractor performance when delivering or removing waste containers. Key inspection points include verifying the contractor's arrival time, equipment condition, container size, presence of plastic liner and tarp cover (for ROB), empty weight ticket, container appearance and integrity, equipment functionality, and overall safety of work performance. The checklist requires the government representative to mark "Yes", "No", or "N/A" for each criterion and provides space for documenting any noteworthy defects or comments, with a requirement for the driver to initial any noted defects to prevent container rejection.
