DIVISION 01 00 00 - GENERAL REQUIREMENTS
SECTION 01 10 00 - SUMMARY
PART 1 - GENERAL
The purpose of this project is to abate hazards posed by three abandoned coal mine features located in
Pierce County, south of the town of Carbonado, Washington. The features are designated as:
◼ CARB-05. In March 2023, the feature consisted of a large funnel shaped opening about 21 feet wide, at the base of a slope adjacent to the Carbon River. The opening continued into a main slope that was partially blocked with a large boulder.
◼ CARB-06. In March 2023, the feature consisted of an elliptical subsidence about 10 feet long, 5 feet wide, and 5.5 feet at its deepest point. The subsidence is possibly connected to a vertical opening but the entry is blocked at the surface by vegetation and debris.
◼ MM-01. In March 2023, the feature consisted of an elliptical subsidence approximately 18 feet long and
16 feet wide. On the northeast side of the subsidence was an approximately 4-foot-wide circular opening connected to open workings that continued at a downward dip between about 30 and
35 degrees.
Maps showing the feature locations are included on Plans C-1 though C-4. Work will consist of reclaiming the mine features, as summarized in Section 1.2 and as specified herein and shown on Plans C-5 through
C-13. When the term “site” is used herein, it collectively refers to the CARB-05, CARB-06, and MM-01 work sites, access routes, and overall project area.
Important: The extent, depth, and stability of historic mine workings in the project vicinity are unknown.
The area surrounding the features may contain unstable mine workings that may be prone to sudden collapse, leading to surface subsidence or collapse that could pose a risk to construction equipment and personnel. The Contractor is responsible for all site safety and should choose construction equipment and methods accordingly.
1.1 Mine Feature Description and Location
Feature CARB-05
Description: In March 2023, Feature CARB-05 consisted of a large funnel shaped opening about 21 feet wide near the base of a slope adjacent to the Carbon River. The opening continued into what appeared to be a main slope with a dip of about 40 degrees to the northeast. There is a large boulder partially blocking the main slope about 21 feet down from the entry. There is an approximately 7-foot-long rock overhang above the entry that contains loose cobbles/boulders ensnarled in tree roots.
Feature CARB-06
Description: In March 2023, Feature CARB-06 consisted of an elliptical subsidence on a steep sloping hillside that measured approximately 10 feet long, 5 feet wide, and 5.5 feet at its deepest point. The
01 10 00-2 0202993-010 headwall of the feature was composed of a vertical rock face and a berm of mounded soil about 1.5 feet high was present immediately downslope of the subsidence. The subsidence was funnel shaped with the deepest section near the northeast side of the vertical rock face. The floor of the subsidence was covered in vegetation and debris including branches and fallen leaves. The subsidence may be open to vertical workings below the debris. The feature was not apparent when approaching from the upslope trail. Local residents have reported that the opening was “venting dangerous gas”. Haley & Aldrich, Inc. (Haley & Aldrich) did not observe evidence of gases based on physical observations (e.g., odor) or photoionization detector (PID) readings.
Location/Property Owner and Access to Features CARB-05 and CARB-06: To access the CARB features, park at the public parking lot at the corner of Pershing Avenue and Washington State Route 165 (WA 165) in the town of Carbonado, Washington, and walk south along the trail at the south end of the lot. The trail parallels WA 165 for about 0.4 miles before turning WSW. Keep following the flat trail, staying to the left at the first fork for about another 0.2 miles. There will be another small fork in the trail.
From this point, to access Feature CARB-06, take the left fork and continue on the narrow trail that parallels the slope. Continue slowly descending on this trail walking southeast for about 175 feet. At this point, leave the trail and begin walking SE downhill. In about 25 feet, see a pair of 8-inch-diameter cedar trees separated by about 2 feet. Feature CARB-06 is immediately downhill of these trees and partially covered by debris. The feature is potentially an open vertical shaft that is nearly invisible from above. Stay at least 5 feet to the right or left of the trees while traveling downhill to avoid accidentally walking/falling into the feature.
To access Feature CARB-05, walk back uphill to the trail at the top of the slope. Continue slowly descending on this trail walking southeast for about 375 feet. At this point, you will reach a large gully. Cross the gully aiming for an 8-foot-diameter cedar tree on the opposite side. From the tree, descend towards the river staying to the southeast (right) of the stream that passes through the gully. After walking about 100 feet downhill, you will see a large overhanging rock ledge on the other side of the stream. Cross the stream and walk towards the overhang. The feature is located below the overhanging rock ledge northwest of the stream and about 115 feet uphill from the river.
The general access route from Carbonado to the features is shown on Plans C-1, C-2, and C-3. There is motorized vehicle access to within approximately 1,500 feet of Feature CARB-05 and 700 feet of Feature
CARB-06.
Feature MM-01
Description: In March 2023, Feature MM-01 consisted of an approximately 18-foot-long by 16-foot-wide elliptical subsidence with an approximately 4-foot-diameter circular opening in the northeast end of subsidence. The feature is located upslope of the Melmont Ghost Town Hiking Trail. The opening extends downward at an approximate dip to the north of 30 to 35 degrees and was open for at least 25 feet from the base of the general subsidence. Visual observations indicate the opening extended further than 25 feet. This feature exhibited signs of active failure with exposed bare rock and ferns hanging off the steep northeast headwall of the feature.
Location/Property Owner and Access to Feature MM-01: From the public parking lot at the corner of Pershing Avenue and WA 165 in the town of Carbonado, Washington, drive 3 miles south along WA 165 and cross the Fairfax bridge. Park in the small lot on the east side of the road (west of the river). Walk back across the bridge and cross over the guardrail onto a steep trail that moves downhill along the bridge
01 10 00-3 0202993-010 trestles (east side of the river). At the base of the decent trail, turn left (south) on the flat walking trail that parallels the river. Walk about 0.2 miles to a small stream crossing. At this point, leave the trail and begin walking uphill NNE staying to the north of the stream. Continue in this direction for about 185 feet to Feature MM-01.

The Contractor shall be paid for the number of bag sets used on site per the unit price and as counted by the OSMRE Field Representative. The quantity of PUF required is the best estimate of OSMRE and is based on limited information. The actual quantity of PUF to be placed shall be determined in the field.
No payment shall be made for unopened bag sets. Once a bag set is opened, its entire contents must be used before another bag set may be opened. The OSMRE COR must authorize the opening of all bag sets; no payment will be made for bag sets opened without prior authorization from OSMRE. If PUF is wasted for any reason, or if PUF is placed inappropriately, or does not meet specifications, payment to the Contractor will be adjusted as deemed appropriate by OSMRE.
Mobilization and demobilization costs associated with “Polyurethane Foam (PUF) Placement –
Feature MM-01” shall be included in bid item No. 140, “Mobilization, Site Preparation, and Site
Access – Feature MM-01”, and bid item No. 190, “Demobilization and Site Restoration –

Payment for “Steel Mesh Reinforced Concrete Veneer – Feature MM-01” shall be at the contract unit price per cubic yard of concrete and shall include full compensation for all materials, equipment, and labor necessary to furnish and place the steel mesh and concrete, as specified and shown on the plans. Limits of the concrete placement shall be as indicated on the plans, or as determined by OSMRE in the field at the time of construction. Only the volume of concrete placed within the limits determined by OSMRE will be paid for; no payment shall be made for concrete placed beyond these limits.
01 20 00-7 0202993-010
The volume of concrete needed is based on the estimate of OSMRE; the volume actually required may vary from the estimate. Mobilization and demobilization costs associated with “Steel Mesh
Reinforced Concrete Veneer – Feature MM-01” shall be included in bid item No. 140, “Mobilization, Site Preparation, and Site Access – Feature MM-01”, and bid item No. 190, “Demobilization and Site Restoration – Feature MM-01.”
For concrete mixed on site, volume measurements shall be based on the recorded number of cement/redi-mix sacks and quantity of water per sack mixed and placed on site, as counted by the
OSMRE COR.
OSMRE shall be liable for the cost of any unused structural concrete as long as:
◼ OSMRE approves the amount of concrete the Contractor proposes to mix on site prior to mixing and placing; and
◼ Failure to place the concrete is solely the result of site conditions, such as the required volume of concrete.
Payment shall not be made for concrete lost to failure of the Contractor’s mixing equipment or placing equipment, or failure of the Contractor to place an adequate amount of PUF fill.
The Contractor shall be solely liable for the cost of all concrete that is not placed within the time specified in Section 03 30 00 for reasons such as failure or poor performance of the Contractor’s equipment, insufficient crew size, or poor direction by the Contractor’s superintendent.
Note that OSMRE, at its sole discretion, may elect to change the amount of concrete placed from the estimate, depending on conditions encountered as the work proceeds.
