SECTION 1
GENERAL
1.1 SCOPE OF WORK: The contractor shall provide all management, tools, supplies, equipment, transportation and qualified personnel necessary to maintain and repair gym equipment at Aviano Air Base, Italy in accordance with (IAW) this Performance Work Statement .The contractor shall perform twice a month preventive maintenance, necessary repairs of equipment and unscheduled service maintenance in compliance with the manufacturer's recommendations/owner's manual and standard practices at Aviano Air Base, Dragon Fitness Center, Bldg 1405 and Bldg 1430 Wyvern Fitness Center.
The contractor shall be responsible for repair and replacement of all worn or failed components and parts and doubtful components that are approaching the point of failure.
Replacement parts shall be of like or current design to the existing fitness equipment.
Contractor supplied services are to include, but are not limited to the list provided in Section 3 Preventive Maintenance Schedule Common Guidelines.
1.1.1 SCHEDULED PREVENTIVE MAINTENANCE: Contractor will perform inspections and maintenance twice a month on each piece of equipment. Repairs will be performed as determined upon inspection or as needed. Inspections will be scheduled Wednesday’s of every 1st and 3rd week of the month.
1.1.2 UPHOLSTER/RE-COVERING: This service is included in the twice a month routine maintenance service. Pads need to be identified for replacement or re-upholstering during normal maintenance visit and replaced on the visit following their arrival. Contractor is responsible for costs associated with off-site reupholster of pads.
1.1.3 UNSCHEDULED MAINTENANCE SERVICES CALLS: In the event an unforeseen emergency develops and a repair cannot wait until the next scheduled visit, the contractor shall provide, three yearly service calls on an emergency basis, to restore the equipment to serviceable condition. The contractor will be required to respond within 48 hours Monday through Friday, excluding weekends and holidays after notification by the Mission Owner (government) of a required emergency service and will be allowed a maximum of an additional 48 working hours to restore the failed unit to serviceable condition.
1.1.4 CONTRACTOR SUPPLIED ITEMS: The contractor shall furnish all labor, transportation, tools, equipment, materials, and parts for on-site and off-site repairs as required. These are considered tools of the trade and costs of the contractor. The contractor shall keep on hand basic items such as nuts, bolts, cables, clips, and attaching supplies available should a repair be required. The contractor will notify in writing via email or maintenance log if any equipment is expected to be out of service greater than 10 business days due to awaiting parts. Additionally, items such as bicycle seats, handle
Page | 4 bars, belts for treadmills, various display consoles, pedals and straps will be ordered when necessary and are considered part of routine maintenance.
1.1.4.1 NON-ROUTINE PARTS: The contractor shall acquire parts and materials required for the performance of this contract. The contractor shall purchase parts and materials at the lowest price obtainable. All discounts and rebates shall be credited to the government.
Contractor shall order and provide all parts required for repair of equipment NLT 24 hours after the routine twice a month service call. . The contractor shall request in the parts order the delivery to Aviano APO address (detailed 31 FSS APO address shall be provided to contractor after award). Contractor will provide Fitness facility manager or established POC with an estimated timeline for arrival of parts once parts are ordered.
Contractor shall ensure that all parts and materials used will be new and conform to the original equipment manufacturers specifications. Rebuilt assemblies or subassemblies may be used to repair the equipment if the item is standard and it is a standard industry practice. Rebuilt unit must carry the same warranty as a new assembly. The Mission Owner approval is required to the purchase and installation of a new assembly.
Compensation for parts will be provided at the contractor’s invoice cost and the contractor will be required to attach a copy of such invoices from their supplier to their bill to the government. Any replacement parts that exceed $500 in cost must be approved by the Mission Owner prior to being purchased by the contractor.
1.1.5 SCHEDULED/UNSCHEDULED MAINTENANCE HOURS OF OPERATION: All services shall be performed during working hours Monday through Friday from 07:30 through 17:30. A list of Holidays and USAFE down days will be provided at the start of contract and each option year.
1.1.6 DOCUMENTING WORK: Contractor will document all work performed and provide a copy to the Fitness Center within two business days upon completion of each service call.
The report shall include the date and time of service call, location of equipment, repairs performed, date and time repairs completed, description parts/materials replaced and actual cost, contract number, and the name of the technician performing the repairs.
Both the contractor and the designated representative must sign the report before the service call can be officially closed.
1.2 PERSONNEL
The contractor shall provide a contract manager and alternate who shall have full authority to act for the contractor on all contract matters which are relating to the daily operation of this contract. The on-site contract manager and alternate shall understand English and be able to read, write, and speak English.
1.2.1 Prior to the performance start date, the contractor shall notify the contracting officer and the Mission Owner in writing of the names of the contract manager and the alternate(s) and the telephone numbers where they can be reached. The contractor shall notify the contracting officer and Mission Owner of any change in the contract manager or the alternate within five days after the change.

1.2.2 Employees. The selection, assignment, reassignment, transfer, supervision, management, and control of contractor employees in performance of this PWS are the responsibility and prerogative of the contractor. However, the contractor shall comply with the general intent and specific policies set forth in the PWS and in regulations of Aviano AB concerning conduct of employees as referenced herein. When directed by the contracting officer or Mission Owner, the contractor shall remove from performance on the contract any person identified as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population. All employees must be able to understand English and to read, write, and speak English fluently.
1.2.3 Contractor personnel shall present a neat appearance and be easily recognized. Employee clothing shall be safe for all operations involved in the contract performance and be within common limits and tastefulness. Wear of military clothing is not permitted.
1.2.4 The contractor shall ensure employees have appropriate, current and valid professional certifications from manufacturer to perform maintenance on the listed equipment.
1.2.5 The contractor shall NOT employ any other person who is an employee of the United States Government if the employment of that person would create a conflict of interest, nor shall the contractor employ any employee of the United States Air Force (USAF), either military or civilian, unless such person seeks and receives approval in accordance with Joint Ethics Regulation DOD 5500.7-R. The contractor shall not employ any employee of the USAF if such employment would be contrary to Air Force politics and guidelines.
1.2.6 The contractor is cautioned that off duty active military personnel hired under this contract may be subject to permanent change of station, change in duty hours, or deployment. Military reservists and National Guard members may be subject to recall active duty. The abrupt absence of these personnel could adversely affect the contractor’s ability to perform; such absence shall not constitute an excuse for nonperformance under this contract.
1.3 SECURITY REQUIREMENTS
1.3.1 Contractor shall be responsible for obtaining all necessary cards, passes, badges, buttons, decals, or other items required for access to the areas in which the work will be performed. Contractor will be required to apply for vehicle and individual identification media at the Security Forces Squadron, Pass and ID Section. Upon completion or termination of the contract or an individual’s employment on the contract, the contractor shall retrieve all identification and access media issued and surrender it to the Security Forces Squadron, Pass and ID Section.
1.3.2 Contractor shall comply with all Aviano AB policies regarding entry to the installation, rules of the road, compliance during contingency operations, and photography. The contractor’s employees are authorized entry to the installation for the purpose of work only and are not authorized in other facilities, areas, or buildings without proper prior authorization.

1.3.3 Contractor shall ensure the areas in which services are performed are cleaned up and all items accounted for upon vacating it.
1.3.4 QUALITY ASSURANCE: The Government shall inspect and evaluate the contractor’s performance to ensure services are received in accordance with requirements set forth in this contract. The fitness center manager/Noncommissioned Officer in Charge (NCOIC) shall inspect by watching actual task performance, physically checking an attribute of the completed task, checking a management information report, investigating customer complaints, conferring with facility managers, or otherwise inspecting the task or its results to determine whether or not performance meets the standards contained in this PWS. The fitness center manager/NCOIC will use the contractor’s work schedule or modified version thereof, to record surveillance results.
Results of the surveillance then become the official US Government record of the Contractor’s performance.

2.1.2 SECURITY POLICE AND FIRE PROTECTION: Emergency services are provided through the base by calling the emergency number at 911.
2.1.3 FORMS AND PUBLICATIONS: Government publications and forms required in the performance of this contract shall be made available to the contractor.
