About this file

This is a Past Performance Questionnaire template issued by the Army Contracting Command (ACC) Aberdeen Proving Ground – Huachuca Division for solicitation W9128Z-20-R-0001. The document is designed to collect structured feedback from government officials and contracting personnel regarding a contractor's or teaming partner's performance on previous contracts or programs.

The questionnaire requires evaluators to assess contractor performance across six primary elements using a six-level rating scale: Exceptional (E), Very Good (V), Satisfactory (S), Marginal (M), Unsatisfactory (U), and Not Applicable (N). The assessment categories include Quality of Product/Service, Schedule, Cost Control, Business Relations, Management of Key Personnel, and Subcontractor Management. Evaluators must provide narrative comments supporting their ratings and address termination history, adherence to technical and management approaches, overall contractor strengths and weaknesses, and whether they would recommend re-award. The questionnaire also collects contractor identification information, evaluator contact details, contract metrics, and product/service descriptions. Completed questionnaires must be submitted to Ms. Adelle Kostur at the ACC Huachuca Division no later than the proposal due date, with submission options via mail or email to facilitate source selection evaluation.

2. SCHEDULE Assess the timeliness of the contractor against the completion of the contract, task order, milestone, delivery schedules, and administrative requirements (e.g., efforts that contribute to or affect the schedule variance). This assessment of the Contractor’s adherence to the required delivery schedule should include the Contractor’s efforts during the assessment period that contribute to or affect the schedule variance. This element applies to contract closeout activities as well as contract performance.
