About this file

This document is a template for a Certificate of Data Return and Destruction required by the Office of the Comptroller of the Currency (OCC).

The certificate serves as a formal attestation that all OCC-provided data and derivative files have been returned or destroyed in compliance with NIST SP 800-88 Guidelines for Media Sanitization standards. The document requires completion by an authorized Data Custodian representative who certifies that all copies of specified data files have been submitted for destruction and deleted from the contractor's systems, with backup tapes to automatically rewrite within a specified number of days. A corresponding IT Representative must also certify that data has been deleted from company systems in accordance with applicable standards. The template includes a table for documenting data file descriptions and the timeframe between creation and last modification or access dates. The form requires signatures and dates from both the Data Custodian and IT Representative, along with project title and company name information. No data files or partial files may be retained unless expressly authorized in writing by the data provider.
