About this file

This is a Past Performance Questionnaire template designed to evaluate contractor performance on projects similar in scope, size, and complexity to the U.S. Customs and Border Protection (CBP) Mobile X-Ray Vehicles IDIQ contract. The questionnaire is intended to assess offerors' qualifications and capability to successfully perform the contract requirements outlined in the Statement of Work.

The questionnaire contains six sections requiring contractors or their references to provide detailed information: general contractor and project information including contract value and period of performance; project description with focus on relevance to Mobile X-Ray Vehicles and Non-Intrusive Inspection (NII) or Biometrics Entry/Exit (BEE) technologies; performance ratings across nine evaluation areas using a five-point scale (Excellent, Very Good, Satisfactory, Marginal, Unsatisfactory); identification of key strengths and weaknesses; recommendations for future similar work; and certification of accuracy. Completed questionnaires must be submitted to the CBP Contract Specialist, Christopher J. Shaw, at christopher.j.shaw@cbp.dhs.gov. The template requests evaluation of critical performance factors including schedule adherence, budget compliance, deliverable quality, technical expertise, subcontractor management, communication effectiveness, responsiveness, problem-solving capability, and regulatory compliance.
