About this file
This is a Contractor Certification document for immigration and employment eligibility compliance required as Attachment D5 to a federal contract opportunity. The certification mandates that contractors comply with the Immigration and Nationality Act of 1952 and related laws enforced by Homeland Security, Immigration and Customs Enforcement, and the U.S. Department of Labor. Contractors are explicitly prohibited from knowingly employing, contracting, or subcontracting with illegal aliens or foreign nationals whose non-immigrant status is in violation or who have failed to maintain compliance with the terms and conditions of their U.S. admission.
The contractor must comply with all E-Verify requirements consistent with Executive Order 12989 and related amendments, as well as applicable Federal Acquisition Regulations. The contract relates to phlebotomy staffing services for the Michael E. DeBakey VA Medical Center in Houston, Texas, requiring 33,280 phlebotomist hours annually with an optional increase of 11,050 hours as needed. Non-compliance with these requirements may result in prohibition of non-compliant foreign nationals from the contractor's place of business and termination of the contract for breach. Contractors are required to obtain similar certifications from their subcontractors. The certification must be signed and dated by an authorized representative and constitutes a legal declaration subject to 18 U.S.C. 1001 regarding false certifications.
