About this file
This is a Performance Work Statement (PWS) for phlebotomy services to be provided at a Department of Veterans Affairs Medical Center (VAMC). The contract period runs from January 7, 2020 through December 31, 2028, with an anticipated start date in Fiscal Year 2020.
The PWS establishes five core performance measures with specific quality standards and surveillance methods. The Personnel measure requires qualified phlebotomy personnel to be available and on-site as needed, monitored through random inspections and attendance sheets at a 100% acceptable quality level. Shift Coverage requires the contractor to maintain adequate staffing to cover phlebotomist absences at 100% standard. Phlebotomist Duties require effective and courteous patient services with no valid customer complaints, measured at 98% acceptable quality through validated user complaints. Duties/Workplace Safety requires proper patient identification using two approved identifiers and immediate reporting of patient safety incidents (within 24 hours), also at 98% standard using direct observation methods. Mandatory Training covering privacy, confidentiality, and HIPAA compliance must be completed annually with zero breaches of policy, monitored at 100% through contractor-provided training evidence and violation reports. All performance measures include favorable contractor performance evaluation as the incentive mechanism. The contractor must also maintain OASC and COR documentation, provide quarterly performance assessments, and ensure contractor personnel meet specific experience and professional credentials as outlined in the detailed statement of work requirements.
