The State makes no guarantee regarding estimated quantities, as actual usage may vary substantially, and the resulting contract will not specify fixed quantities.

NB 26-88274 DNR Fish Feed
INSTRUCTIONS
1. Please populate the YELLOW SHADED CELLS in this workbook. Any attempt to manipulate the format of the Bid List document will put your proposal at risk of disqualification.
2. Enter YES/NO to all minimum specification requirements listed on the Minimum Requirements tab. Respondent must review the Specification Requirements document (Attachment H) for a complete listing of required specifications.
3. Pricing provided shall be the UNIT PRICE per pound.
4. Pricing must be ALL INCLUSIVE, which includes all shipping, freight, delivery, or destinations fees. Additional charges will not be accepted (on order size of 24,000 pounds are more).
5. The evaluation will be conducted on lines 1 through 4. Prices per pound must be based inclusive of all charges including shipping based on a minimum quarterly order size of 24,000 pounds. If smaller orders are needed between quarterly order, shipping will be allowed on orders smaller than 24,000 lbs.
6. Estimated quantities of items, based on historical data, have been provided for reference purposes only. The State does not guarantee to meet the estimated quantities indicated. While respondents should consider this information, please understand that it does not represent a commitment by the State and that actual usage may be substantially more or less, as the resulting contract will not specify any fixed quantities.
7. After completing the Bid Cost worksheet, use the Total Bid Cost from cell E16 on the Bid Cost tab to complete the Total Bid Amount on the IVOSB Subcontractor Commitment Form.
8. Respondent must also provide a detailed specification sheet and quote (on company letterhead) with their bid submission.
9. Return WORKING Excel file with bid submission. Bids submitted without a working copy of this Excel file may be deemed unresponsive.

Minimum Requirements

MINIMUM REQUIREMENTS

The minimum specification requirements are outlined in the Specification Requirements document (Attachment H). All items bid must meet or be comparable to these requirements. Failure to include any information requested below and in the Specification Requirements document may result in the removal of a respondents bid from consideration. If any minimum requirement or specification is considered outside of standard specifications, please provide an explanation for review by the State.
Please provide a Yes or No response in the yellow-shaded area below (column D). If YES is entered, the item(s) must be in compliance with the specifications as written. If you are entering NO for any reason, please provide an explanation in the space provided (column E). Failure to provide a response may result in the bid being disqualified.

Please populate the yellow-shaded cells with UNIT pricing (per pound) for each item listed below. The green-shaded cells will auto-populate. The unit price should be reflective of the entire cost of the fish feed. Pricing must be all inclusive, which includes all shipping, freight, delivery, or destinations fees. Additional charges will not be accepted (on order size of 24,000 pounds are more).
