About this file

This is a Proposal Cover Sheet for a Mini-RFP issued by the Oregon Department of Transportation (ODOT) regarding Architecture & Engineering (A & E) and Related Services for the Hillsboro/Beaverton OR8/OR10 ADA Ramps project. The solicitation is open to eligible Tier B Price Agreement holders, with proposers required to include a primary contact name, email address, and telephone number on their submission. A minimum requirement specifies that at least one Oregon Registered Professional Engineer must be part of the proposal team. Specific response dates, due dates, site visit details, and bidder conference information are not included in this cover sheet document itself but are referenced as being contained in the broader Mini-RFP and the sample Work Order Contract (WOC) included as Attachment C.

The cover sheet establishes mandatory certifications and representations that proposers must attest to, including confirmation of receipt of all addenda, truthfulness of proposal contents, compliance with non-discrimination requirements in subcontracting, and possession of written policies preventing sexual harassment and discrimination for contracts anticipated to exceed $150,000. Proposers must also certify they have no undisclosed liquidated or delinquent debt owed to the State of Oregon, accept all terms and conditions of the Mini-RFP, and make required Conflict of Interest disclosures. An authorized representative must sign and approve the cover sheet to bind the proposer and certify all representations contained in the proposal document.
