About this file
This is a Mandatory Criteria Certification Form issued by the Florida Department of Financial Services for the Statewide Response Team Vehicles Equipment Installation solicitation. The form requires respondents to certify compliance with nine mandatory criteria, including authorization to respond on behalf of the organization, disclosure of conflicts of interest, registration with the Florida Department of State, and absence of contract terminations within the past year. Additionally, respondents must certify their capability to perform services within the contiguous United States and within 500 miles of Ocala, Florida; remove and install non-walk-in investigations support units on new F-550 chassis either in-house or through subcontractors; and service SLERS equipment using qualified, certified technicians pre-approved by the SLERS security manager. A mandatory requirement is that at least one representative of the respondent attended the pre-response conference in person on May 19, 2026, with attendance verified on the Department's sign-in sheets.
The form includes vendor certification requirements addressing boycotts of Israel, scrutinized companies lists, suspended vendor status, E-Verify registration, and compliance with Florida disclosure requirements. If the contract value exceeds $1 million, vendors must certify non-involvement with Sudan, Iran terrorism sectors, Cuba, or Syria. Respondents must complete supplementary attestation forms including the Foreign Country of Concern Attestation Form (PUR 1355) if the contract grants access to personal identifying information, the Common Carrier or Contracted Carrier Attestation Form (PUR 1808) if applicable, and certifications regarding non-use of coercion for labor and non-use of forced labor in commodity production. False information on the certification form prior to award renders the respondent non-responsive; false information discovered after award may result in contract termination and liability for re-procurement costs. Annual resubmissions of attestation forms do not require written contract amendments.
