About this file
This is a Contractor Past Performance Data Sheet template designed to collect and evaluate contractor performance information for the DLA Disposition Services Ft. Lewis MHE Repair and Maintenance solicitation (SP451026Q1022). The form is completed in two parts: Part I is completed by the quoter and includes contract details such as contract number, point of contact information, detailed work description, dollar value, subcontractor names and work descriptions, and any quality, delivery, or price problems encountered along with corrective actions taken. Part II is completed by the government or commercial respondent and returned directly to DLA Disposition Services for performance evaluation purposes.
The evaluation criteria in Part II assess contractor performance across five primary categories using a five-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory): technical performance and quality of product/service; management and key personnel effectiveness; schedule compliance and timeliness; small business and socioeconomic subcontracting utilization; and compliance with regulatory requirements including environmental, hazardous waste, bio-based product utilization, and EEO compliance. Respondents provide narrative comments supporting each rating and conclude with an overall performance assessment and a binary recommendation regarding future contract awards. This template is part of the evaluation process for the small business set-aside, firm fixed price IDIQ contract with a base period plus two 12-month option years.
