STATEMENT OF WORK (SOW)

PURPOSE

The purpose of this project is to provide engineering and construction services (design-build) for plumbing throughout the campus grounds of the Carl T. Hayden VA Medical Center.

PERIOD OF PERFORMANCE (POP):

Complete all work including submittals, reviews, and quality control within 455 calendar days from Notice to Proceed (NTP) (150 calendar days for design which includes all reviews, submissions, etc./305 calendar days for construction). The successful contractor and all sub-contractors shall have the capacity to perform the construction on time without delay.

Site Visits:

A site visit shall be conducted prior to bidding in order to clarify any RFIs prior to submitting bid packages.

Review and vetting of existing documentation and site conditions shall be accomplished by site visits conducted during the design phase. Conduct site visits, as required, to fully comprehend and assess the current condition of any equipment in question.

Site Visits shall be done prior and during the entire project to avoid delays and corrections. At a minimum the Engineering Firm shall do a site visit approximately every 15 calendar days or at timing requested by the VA to ensure installation is per the drawings (Total of approximately 23 minimum site visits).

Documentation of all site visits shall be submitted in writing to the VA Contracting Officer and Contracting Officer’s Representative (COR) within one (1) week after each visit for review and inspection of progress including final tests and inspections.

SCOPE OF WORK:

The Design-Build contractor shall provide all site investigations, design analysis, design, design specifications and drawings, construction documents, materials, labor, equipment, supervision, testing, shop and manufacturing drawings, submittals, submittal registry/index, and construction to perform the requirements of this project in accordance with this SOW. The General Contractor shall partner with the Designer of Record (DOR), and other required design disciplines to perform the design and construction work required to provide all requirements stated herein. The contractor shall coordinate with Government Representatives under the direction of the Contracting Officer and assigned representatives of the contract. The contractor shall perform a thorough inspection of the site/site plan against the requirements of this Request for Proposal (RFP) prior to submitting a proposal. The Contractor shall be responsible for verifying conditions, quantities, loadings, and dimensions in this document and appendices in preparation of the proposal. Construction will commence under the direction of the Contracting Officer after the completed design is reviewed and approved, in writing that the design has demonstrated compliance with the contract requirements. The Certification and Accreditation (C&A) requirements do not apply, and a Security Accreditation Package is not required.

Coordination and approval if any with the City of Phoenix for the work at the connection of the two mains located on E. Indian School Rd and 7th St. is the responsibility of the AE/Contractor.

This project is to replace the entire existing exterior facility water piping around the main facility buildings (which is “L” shaped) due to failures and deterioration (entire existing water loop). The project, at minimum, will consist of utilizing two (2) of the city water line tie-ins and running piping with shut-off valves in a proper valve box (sized appropriately to support current and future expansion of the VAMC) to a central location specified by the Phoenix VAMC where a small structure must be built to house the VA

Required Water Treatment Equipment for Domestic Water with proper ventilation and temperature control in order to maintain the equipment and be blast resistant since water supply is Critical Infrastructure (PG-18-10 Design Manual - Physical Security and Resiliency) https://www.cfm.va.gov/til/index.asp. Disconnect the water line from the parking garage to the loop so that the water line is only for the parking garage.

After the treatment, pipes shall be installed with shutoff valves to buildings around the main facility buildings (which is “L” shaped) within housings creating a loop and to tie-in to all VAMC structures (approximately 8 tie-ins). Shutoff valves shall be placed at locations to be determined and would be utilized in an emergency.

Replace and/or install the following: (C900 or Equivalent)

1. Approx. Two Thousand (2,000) Linear Ft – 12” pipe.

2. Approx. Two Thousand (2,000) Linear Ft – 10” pipe
