About this file

This is a Schedule of Items pricing template for a Request for Quotation (RFQ) for the Cisco Branch Ontonagon River Bridge Replacement project located on the Ottawa National Forest Watersmeet Ranger District in Ontonagon County, Michigan. The project involves removal and replacement of an existing concrete bridge with a 70-foot single span concrete I-girder bridge featuring a concrete deck, concrete abutments, and H-pile foundations. Contractors must provide firm fixed prices for all 40 line items to be eligible for award, including mobilization, construction survey and staking, erosion control measures, excavation and embankment work, structural components (H-piles, prestressed concrete I-girders, reinforcing steel, and structural steel), bridge appurtenances (railing, bearings, expansion joints, and waterproofing), approach work (guardrails and curb/gutter), and site restoration (seeding, mulching, and erosion control products). Quantities range from small items such as four object markers to larger quantities including 282 feet of H-pile, 143 cubic yards of structural concrete, 274 feet of prestressed concrete I-girders, and 23,351 pounds of epoxy-coated reinforcing steel.

This acquisition is set-aside as a Total Small Business Set-Aside with a small business size standard of $45,000,000 under NAICS Code 237310 (Highway, Street, and Bridge Construction). The contracting officer requires contractors to have active registrations at sam.gov to be eligible for award and to enter prices for all line items directly into the provided schedule. Payment will be made on actual work performed unless otherwise noted, with bond premiums to be included under the Mobilization line item rather than added to the contract price. All work necessary to complete the project not explicitly listed is considered incidental to the scheduled line items. The official source of solicitation information is sam.gov under solicitation number 12445527Q0001, issued by the Department of Agriculture Forest Service.

Schedule Notes

1. One Firm Fixed Price (FFP) contract is anticipated from this RFQ. Contractor must enter prices for all line items to be eligible for award. Type prices directly into the schedule provided (if able) to promote readability.

2. Contractor must have an active registration at sam.gov to be eligible for award. Note that a status of "ID Assigned" is not a fully activated registration.

3. Payment will be made on actual work performed as described in FP-14 109.01 unless otherwise noted. Any work necessary to complete this project that is not listed in the schedule of items, is considered incidental to the line items shown. If you believe a pay item is missing from the schedule, contact the contract specialist listed in Block 8 of the SF-1442.

4. Payment for bond premiums in accordance with Clause 452.232-71, Progress Payments for Commercial Construction Contracts, shall not be in addition to the contract price. Include bond payments under 151.01 Mobilization. The price of any contract awarded from this RFQ will not be increased to accommodate a contractor's failure to account for bonding premiums in their quote.

5. Quantities for aggregate are compacted in place volumes.

6. The official source of information about this solicitation is sam.gov. If you obtained these materials from any other source, search for solicitation 12445527Q0001, on sam.gov, to make sure you have the most current information before contacting the contract specialist with any questions.
